目的探讨2型糖尿病肾病(DN)早期检测胱抑素C(Cys C)和同型半胱氨酸(Hcy)的意义。方法将60例2型糖尿病(T2DM)患者按DN诊断标准分为DN组与非糖尿病肾病组(NDN)。选择40例健康人作为对照组(NC)。同时检测三组受试者糖化血红蛋白(HbA1c)、肌酐(Cr)、尿素氮(BUN)、尿微量清蛋白排泄率(UAER)、Cys C和Hcy,并对Cys C和Hcy进行相关统计学分析。结果三组HbA1c、Cr、BUN、UAER、Cys C和Hcy水平比较差异有统计学意义(P<0.01)。DN组Cys C和Hcy水平高于NDN组和NC组(P<0.01);NDN组和NC组Cr、BUN、UAER、Cys C和Hcy比较差异无统计学意义(P>0.05);DN组和NDN组分异与NC组的HbA1c水平差异有统计学意义(P<0.05),而DN组和NDN组的HbA1c水平差异无统计学意义(P>0.05)。Cys C与Hcy变量间存在显著正相关(r=0.782,P<0.05;r=0.912,P<0.01)。结论 Cys C和Hcy是早期诊断2型糖尿病肾病肾损害的重要指标。
Objective To investigate the early detection of cystatin C (Cys C) and homocysteine (Hcy) in type 2 diabetic nephropathy (DN). Methods Sixty patients with type 2 diabetes (T2DM) were divided into DN group and non-diabetic nephropathy group (NDN) according to the diagnostic criteria of DN. Forty healthy people were selected as the control group (NC). At the same time, HbA1c, creatinine, BUN, UAER, Cys C and Hcy in three groups were tested simultaneously, and the correlation between Cys C and Hcy . Results There were significant differences in the levels of HbA1c, Cr, BUN, UAER, Cys C and Hcy between the three groups (P <0.01). The levels of Cys C and Hcy in DN group were significantly higher than those in NDN group and NC group (P <0.01). There was no significant difference in Cr, BUN, UAER, Cys C and Hcy between NDN group and NC group (P> 0.05) The difference of HbA1c level between NDN group and NC group was statistically significant (P <0.05), but no significant difference was found between DN group and NDN group (P> 0.05). There was a significant positive correlation between Cys C and Hcy (r = 0.782, P <0.05; r = 0.912, P <0.01). Conclusions Cys C and Hcy are important indicators for the early diagnosis of renal damage in type 2 diabetic nephropathy.
