目的 :考察合并用环孢素或者FK506时,霉酚酸有限采样法公式拟合的差异。方法 :采用高效液相法检测20例患者服用MMF后0~12h内不同时间点MPA浓度,其中10例患者合并用CsA,另外10例患者合用FK506,分别拟合两组患者MPA的有限采样法公式以及总体样本的有限采样法公式,比较其差异。结果 :单一样本拟合时,单个采血点的浓度与AUC的相关度均较差。多点拟合时,总样本中,3,1.5,6,8h四个时间点血浆浓度拟合效果最好,其r2为0.989;而CsA组中,0.5,1.5,3三个采血点和0,0.5,1.5,3四个采血点拟合效果较好,r2分别为0.991和0.993,FK506组,0,3,4,8四个采血点拟合效果拟合效果较好,r2为0.993。CsA组与FK506组拟合差异较大。结论 :合并用药对MPA有限采样法的公式影响较大,有限采样法拟合应注意合并用药对药物药动学的影响。
OBJECTIVE: To investigate the differences of the fitting formulas for mycophenolic acid limited sampling when using cyclosporine or FK506. Methods: The MPA concentrations of 20 patients at different time points after 0 ~ 12 hours of MMF were measured by high performance liquid chromatography. Among them, 10 cases were combined with CsA and the other 10 cases were treated with FK506. The MPA finite sampling method Formula and finite sampling of the overall sample formula, compare the differences. Results: When the single sample was fitted, the correlation between single blood collection point concentration and AUC was poor. In the multi-point fitting, the best fitting result was obtained at the three time points of 1, 3.5, 6 and 8h in the total sample, with a r2 value of 0.989. In the CsA group, 0.5, 1.5, 3 and 0 , 0.5,1.5,3 four blood collection points fitting better, r2 were 0.991 and 0.993, FK506 group, 0,3,4,8 four blood collection point fitting effect fitting better, r2 0.993. The difference between CsA group and FK506 group was significant. Conclusion: The combination of drugs on MPA limited sampling method has a greater impact on the formula, limited sampling method fitting should pay attention to the impact of the combination of drug pharmacokinetics.
