目的探讨2型糖尿病(T2DM)患者血清8-羟基脱氧鸟苷酸(8-OHdG)与胰岛素抵抗(IR)和颈动脉内膜中层厚度(IMT)的相关性。方法 50例初诊T2DM患者根据IMT分成A组(IMT<1.0 mm,32例)和B组(IMT≥1.0 mm或存在硬化斑块,18例);正常对照组(C组,25例)。检测三组血清8-OHdG水平、体重指数(BMI)、空腹血糖(FBG)、血脂、胰岛素(Ins)、糖化血红蛋白(HbA1c),计算胰岛素抵抗指数(Homa-IR)。结果 A组和B组的血清8-OHdG水平均显著高于C组,且B组较A组显著升高(P<0.05);血清8-OHdG水平与Homa-IR和IMT呈正相关(P<0.05)。8-OHdG、Homa-IR、HbA1c、总胆固醇(TC)为IMT的风险预测因子。结论血清8-OHdG水平增高与IR和颈动脉IMT的增厚相关,提示氧化应激参与了IR的形成及T2DM并发症的发生。
Objective To investigate the relationship between serum 8-hydroxydeoxyguanosine (8-OHdG) and insulin resistance (IR) and carotid artery intima-media thickness (IMT) in type 2 diabetes mellitus (T2DM) Methods 50 cases of newly diagnosed T2DM patients were divided into group A (IMT <1.0 mm, 32 cases) and group B (IMT≥1.0 mm or sclerosis plaque, 18 cases) according to IMT; and normal control group (group C, 25 cases). Serum levels of 8-OHdG, body mass index (BMI), fasting blood glucose (FBG), blood lipid, insulin, HbA1c and Homa-IR were measured in the three groups. Results Serum levels of 8-OHdG in group A and group B were significantly higher than those in group C, and group B was significantly higher than group A (P <0.05). The level of serum 8-OHdG was positively correlated with Homa-IR and IMT (P < 0.05). 8-OHdG, Homa-IR, HbA1c, total cholesterol (TC) were risk predictors of IMT. Conclusions Elevated serum 8-OHdG levels are correlated with IR and carotid IMT thickening, suggesting that oxidative stress is involved in the formation of IR and T2DM complications.
