目的探讨早期应用无创机械通气对慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的治疗效果。方法在常规治疗基础上早期采用双水平正压(BiPAP)S/T 无创通气模式治疗 COPD并Ⅱ型呼吸衰竭患者12例,观察治疗前后动脉血气分析的变化,治疗过程中监测无创经皮血氧饱和度(SaO_2)及脉搏的变化。结果 12例患者经过7~10 d 的治疗后其呼吸频率及心率减慢,气促症状减轻。治疗后与治疗前动脉血气分析比较,酸碱度(pH)、动脉氧分压(PaO_2)升高,动脉二氧化碳分压(PaCO_2)降低,差异均有显著性意义(P<0.05)。结论早期应用无创机械通气治疗 COPD并Ⅱ型呼吸衰竭有确切的疗效,可以有效缓解症状,防止呼吸衰竭进一步加重。
Objective To investigate the therapeutic effect of early noninvasive mechanical ventilation on chronic obstructive pulmonary disease (COPD) with type Ⅱ respiratory failure. Methods Twelve patients with COPD and type Ⅱ respiratory failure were treated with bi-level positive pressure (BiPAP) S / T noninvasive ventilation on the basis of routine treatment. Changes of arterial blood gas analysis before and after treatment were observed. Non-invasive percutaneous oxygen monitoring Saturation (SaO_2) and pulse changes. Results After 12 to 10 days of treatment, the respiratory rate and heart rate of 12 patients were slowed down and symptoms of shortness of breath were relieved. After treatment, compared with the arterial blood gas analysis before treatment, pH, PaO_2 and PaCO_2 were decreased (P <0.05). Conclusion The early application of noninvasive mechanical ventilation in the treatment of COPD and type Ⅱ respiratory failure has the exact effect, which can effectively relieve symptoms and prevent further worsening of respiratory failure.
