目的评价人工骨替代材料是否能通过微创手术快速重建骨强度,并取得安全的愈合效果.方法从2016年8月至2017年2月,共10例良性骨肿瘤病例在我科接受经皮穿刺注射型人工骨再生骨替代材料治疗.10例病理诊断均为骨囊肿.接受治疗的肿瘤部位均位于长管状骨,包括股骨4例、肱骨5例和胫骨1例.通过术后随访,评估植骨部位的骨吸收转化过程和结果,记录患者术后MSTS功能评分和手术相关并发症.结果术后随访平均12.4(8~17)个月.截止最终随访,人工骨转化率平均为98.8%(90%~100%),全部病例愈合良好.8例随访达到1年,转化率均为100%.植骨术后转化率与年龄(P=0.552)、部位(P=0.781)和植骨量(P=0.508)无显著相关性.术后MSTS功能评分平均29.3分.5例术中出现少量人工骨经穿刺道溢出骨皮质外,术后4~5个月外漏人工骨完全吸收,无软组织内成骨.术后未见肿瘤复发、骨折、伤口感染及其它手术相关并发症.结论应用注射型硫酸钙/磷酸钙复合骨移植物,治疗以骨囊肿为代表的良性骨肿瘤,获得满意的临床效果,愈合率高.植骨后可保持较高的骨强度,患者术后功能恢复良好,并发症发生率低.“,”Objective To evaluate whether artificial bone replacement material can rapidly reconstruct the bone strength through minimally invasive surgery and achieve safe healing effects.Methods From August 2016 to February 2017, 10 cases of benign bone tumor were treated by percutaneous injection of the artificial bone substitute in our department.All were diagnosed as bone cysts.All the tumors located in the long tubular bones (4 femur, 5 humerus, 1 tibia).The bone resorption and transformation process, outcomes of the bone grafting were evaluated by postoperative follow-up.Postoperative MSTS score and complications were recorded.Results The average followup was 12.4 months (range:8-17 months).The average transformation rate was 98.8% (range:90%-100%) at the end of follow-up.All cases healed well.Eight cases had been followed up for 1 year and the transformation rate was100%.There were no significant correlations between the transformation rate and age (P=0.552), location (P=0.781), bone graft volume (P=0.508) after bone grafting.The average MSTS score was 29.3.In 5 patients, a small amount of artificial bone spilled out of the bone cortex through the puncture path, which was completely absorbed4-5 months postoperatively.There was no osteogenesis in soft tissues.No recurrence, fracture, wound infection or other complications was noted.Conclusions The injection type calcium sulfate/calcium phosphate compound bone graft can be used to treat benign bone tumors represented by bone cyst.The clinical effects are satisfactory and the healing rate is high.The bone strength can be maintained after bone grafting.The postoperative functional recovery is good and the complication rate is low.
