目的 :观察纤维支气管镜 (纤支镜 )直视下放置镍钛记忆合金支架治疗气管狭窄的可行性和疗效。方法 :选取 12例气管肿瘤引起的气管狭窄患者 ,在纤支镜直视下放置镍钛合金支架 ,12例中有 3例先用自制的气囊扩张器将狭窄段扩张 ,4例使用微波治疗使管腔较为通畅后放置支架 ,5例直接放置。结果 :12例患者在支架放置后呼吸困难均立即缓解。支架放置后第 3、7天复查纤支镜见支架无移位 ,管腔通畅。有 1例术后 15d因消化道出血死亡 ,其余 11例 1月后再次复查纤支镜 ,支架位置无变化 ,管腔通畅。结论 :使用镍钛记忆合金支架治疗肿瘤引起的气管狭窄可以即刻缓解气管狭窄的程度和改善患者的生活质量 ,纤支镜直视下放置气管支架简单、易行、可靠。
OBJECTIVE: To observe the feasibility and efficacy of bronchoscope (bronchoscope) placed nitinol stent under direct vision in the treatment of tracheal stenosis. Methods: Twelve patients with tracheal stenosis caused by tracheal neoplasm were selected. Nickel-titanium stent was placed under direct vision with fiberoptic bronchoscopy. Three of 12 patients first dilated the stenosis with a homemade balloon dilator and four patients were treated with microwave After the lumen is relatively smooth, the stent is placed and 5 cases are placed directly. Results: All 12 patients were relieved immediately after stent placement. Stent placed on the 3rd and 7th days after review of bronchoscopy showed no stent displacement, lumen patency. One case died of gastrointestinal bleeding 15 days after operation, and the other 11 cases were reviewed again after January. The position of the stent was unchanged and the lumen was unobstructed. Conclusion: The use of nickel-titanium memory alloy stent treatment of tumor-induced tracheal stenosis can immediately ease the degree of tracheal stenosis and improve the quality of life of patients, bronchoscopy placement of tracheal stent under direct vision is simple, easy and reliable.
