目的:探讨尿中IL-18的水平与狼疮性肾炎(LN)肾脏病理改变参数之间的关系。方法:应用ELISA法测定19例正常人和55例LN患者晨间自由尿及24h尿中IL-18的水平,应用直线相关分析法分析晨间自由尿及24h尿中IL-18的水平与LN肾脏病理改变的参数,即活动指数(AI)和慢性指数(CI)的相关性。结果:LN组晨间自由尿及24h中IL-18的水平均较正常对照组显著增高。两组晨尿中IL-18的水平分别为:(247.1±317.5)ng/L和(20.3±14.5)ng/L(P<0.001);24h尿中IL-18的水平分别为(192.1±170.1)ng/d和(21.0±3.8)ng/d(P<0.001),晨间自由尿及24h尿中IL-18的水平,均与LN病理改变参数AI呈密切正相关(晨尿:r=0.602,P<0.001;24h尿:r=0.461,P<0.005);但二者均与病理改变参数CI无相关性(P>0.05)。按AI值将LN患者分为高、中、低3个组,各组尿中IL-18的水平差异显著。晨尿中分别为:(69.2±82.7)ng/L、(193.5±106.1)ng/L和(580.7±453.1)ng/L(P<0.001);24h尿中分别为(103.5±141.4)ng/d、(188.8±124.0)ng/d和(333.1±183.2)ng/d。结论:检测晨间自由尿中IL-18的水平很简便,可作为评估LN肾脏病理活动程度的一种好方法。
Objective: To investigate the relationship between urinary levels of IL-18 and renal pathological parameters of lupus nephritis (LN). Methods: The morning morning free urine and 24-hour urinary IL-18 levels in 19 normal subjects and 55 LN patients were measured by ELISA. The correlation between free urine in 24 hours and IL-18 in 24h urine was analyzed by linear correlation analysis. Renal pathological parameters, namely, activity index (AI) and chronic index (CI) correlation. Results: Free morning urine and LN-18 levels in LN group were significantly higher than those in normal control group. The levels of IL-18 in morning urine of the two groups were (247.1 ± 317.5) ng / L and (20.3 ± 14.5) ng / L, respectively. The levels of IL-18 in 24h urine were (192.1 ± 170.1 (P <0.001). The levels of IL-18 in morning urine and urine in 24 hours were positively correlated with AI of pathological parameters of LN (morning urine: r = 0.602, P <0.001; 24h urine: r = 0.461, P <0.005), but both had no correlation with pathological parameters CI (P> 0.05). According to AI value, LN patients were divided into high, medium and low groups. The levels of IL-18 in urine of each group were significantly different. The morning urine was (69.2 ± 82.7) ng / L, (193.5 ± 106.1) ng / L and (580.7 ± 453.1) ng / L, respectively; d, (188.8 ± 124.0) ng / d and (333.1 ± 183.2) ng / d, respectively. CONCLUSIONS: The detection of morning urine IL-18 levels is very simple and can be used as a good way to assess the degree of LN renal pathology.
