评价几种铁参数在铁缺乏症诊断中的价值。方法 连续性收集 90例贫血病人 (缺铁性贫血 36例 ,慢性病贫血 2 3例 ,慢性病伴缺铁性贫血 31例 ) ,做骨髓铁染色、血清转铁蛋白受体 (sTfR)、血清铁蛋白 (serumferritin ,SF)、血清铁 (serumiron ,SI)、总铁结合力 (totaliron bindingcapacity ,TIBC)、转铁蛋白饱和度 (transferrinsaturation ,TS)等检查。应用似然比 (LR)和受试者工作特征 (ROC)曲线分析方法 ,以骨髓铁染色作为金标准对以上的铁参数诊断慢性病合并缺铁性贫血的价值进行分析。结果 在判断慢性病是否合并缺铁性贫血时SF在 2 5~ 44 μg/L时的LR最大 (5 .8) ,STfR >5 0nmol/L时的LR最大(6 .0 ) ;ROC曲线下面积以STfR最大 (0 .84) ,其他依次是STfR/logSF(0 .83)、SF(0 .77)、sTfR/SF(0 .74)、SI(0 .72 )、TIBC(0 .6 8)。结论 ROC曲线与LR对科学评价铁参数诊断价值有重要的意义 ,研究结果对于临床诊断具有重要的参考价值。
To evaluate the value of several iron parameters in the diagnosis of iron deficiency. Methods A total of 90 patients with anemia (iron deficiency anemia in 36 cases, chronic anemia in 23 cases and chronic disease with iron deficiency anemia in 31 cases) were enrolled in this study. The bone marrow iron staining, serum transferrin receptor (sTfR), serum ferritin Serum ferritin (SF), serum iron (SI), total iron binding capacity (TIBC) and transferrin saturation (TS) were measured. The values of the above iron parameters in the diagnosis of chronic diseases with iron deficiency anemia were analyzed by applying the LR method and the ROC curve analysis method with the bone marrow iron staining as the gold standard. Results When the SF was between 5 and 44 μg / L, the maximum LR was found when STfR was more than 500 nmol / L and the area under the ROC curve (0.84), followed by STfR / log SF (0.83), SF (0.77), sTfR / SF (0.74), SI (0.72), TIBC ). Conclusion The ROC curve and LR are of great significance in the scientific evaluation of iron parameters. The research results have important reference value for clinical diagnosis.
