目的探讨糖化血红蛋白(HbA1c)、糖化血清蛋白(GSP)、P选择素(P-sel)及纤维蛋白原(FIB)在监测糖尿病及诊断早期血管并发症患者中的临床价值。方法采用流式细胞术、全自动生化和血凝分析仪分别对146例糖耐量正常者(对照组)、165例糖尿病患者(糖尿病组)进行HbA1c、GSP、P-sel及FIB测定,并对结果进行分析。结果糖尿病组HbA1c、GSP、P-sel及FIB的水平均明显高于对照组(P均<0.05);有血管并发症组HbA1c、GSP、P-sel、FIB的水平又高于无血管并发症组(P均<0.05)。结论检测HbA1c、GSP水平能较好地反映血糖控制水平,对糖尿病的控制有重要的临床应用价值,P-sel及FIB水平显著上升说明糖尿病患者的血小板及血管内皮细胞高度活化、血液高凝程度加剧,是血管并发症形成的危险因素,联合检测对监测糖尿病病情变化及诊断早期血管并发症有重要意义,应予以重视。
Objective To investigate the clinical value of glycosylated hemoglobin (HbA1c), glycosylated serum protein (GSP), P-sel and fibrinogen (FIB) in the monitoring of diabetes mellitus and early diagnosis of vascular complications. Methods The levels of HbA1c, GSP, P-sel and FIB in 146 patients with normal glucose tolerance (control group) and 165 diabetes patients (diabetic group) were measured by flow cytometry, automatic biochemical analyzer and hemagglutination analyzer respectively. Results were analyzed. Results The levels of HbA1c, GSP, P-sel and FIB in diabetic group were significantly higher than those in control group (all P <0.05). The levels of HbA1c, GSP, P-sel and FIB in patients with vascular complications were significantly higher than those without vascular complications Group (all P <0.05). Conclusion Detection of HbA1c and GSP levels can better reflect the level of blood sugar control, which has important clinical value for the control of diabetes. The levels of P-sel and FIB are significantly increased, indicating that the platelets and endothelial cells in patients with diabetes are highly activated and hypercoagulable Aggravating is a risk factor for the formation of vascular complications. Joint detection is important for monitoring the changes of diabetes and diagnosing early vascular complications.
