目的探讨多囊卵巢综合征(PCOS)患者的血清胃生长素(Ghrelin)、阿立新-A(Orexin-A)水平与性激素及糖脂代谢指标水平的关系。方法收集2012年12月-2016年12月该院收治的PCOS患者89例,根据体质指数(BMI)分为肥胖组(BMI≥25 kg/m~2)38例、非肥胖组(BMI<25 kg/m~2)51例;另选取同期在该院治疗的输卵管性不孕患者49例作为对照组。采用酶联免疫吸附法(ELISA)检测患者血清Ghrelin、Orexin-A水平;采用全自动化学发光法测定性激素、糖代谢指标及脂质代谢指标水平。结果肥胖组血清Ghrelin、Orexin-A水平低于非肥胖组及对照组,差异有统计学意义(P<0.05);肥胖组的血清性激素指标黄体生成素(LH)、雌二醇(E2)、睾酮(T)、卵泡刺激素(FSH)及糖代谢指标空腹胰岛素(FINS)、2 h胰岛素(INS 2 h)、空腹血糖(FPG)、2 h血糖(GLU 2 h)水平高于非肥胖组及对照组,脂质代谢指标甘油三酯(TG)、总胆固醇(TC)、游离脂肪酸(FFA)、低密度脂蛋白胆固醇(LDL-C)水平高于非肥胖组及对照组,高密度脂蛋白胆固醇(HDL-C)水平低于非肥胖组及对照组,差异均有统计学意义(P<0.05)。经Pearson检验发现,PCOS患者血清Ghrelin、Orexin-A水平与性激素及糖脂代谢指标水平存在直接相关关系(P<0.05)。结论 PCOS患者体内存在低水平的Ghrelin、Orexin-A,可客观反映性激素及糖脂代谢的变化。
Objective To investigate the relationship between the levels of serum ghrelin and Orexin-A in patients with polycystic ovary syndrome (PCOS) and the levels of sex hormones and glucose and lipid metabolism. Methods Eighty-nine PCOS patients admitted to our hospital from December 2012 to December 2016 were divided into three groups: obese group (BMI≥25 kg / m ~ 2) 38 cases and non-obese group (BMI <25) according to body mass index kg / m ~ 2) 51 cases; the other selected during the same period in the hospital treatment of tubal infertility in 49 cases as a control group. Serum levels of Ghrelin and Orexin-A were detected by enzyme-linked immunosorbent assay (ELISA). The levels of sex hormones, glycometabolism and lipid metabolism were measured by the automatic chemiluminescence method. Results Serum levels of Ghrelin and Orexin-A in obesity group were significantly lower than those in non-obese group and control group (P <0.05). Serum levels of sex hormone LH, Testosterone, FSH, FINS, INS 2 h, fasting blood glucose (FPG) and GLU 2 h were higher than those in non-obese group (TG), total cholesterol (TC), free fatty acid (FFA), and low density lipoprotein cholesterol (LDL-C) levels in the control group were higher than those in the non-obese group and the control group The level of HDL-C was lower than non-obese group and control group (P <0.05). Pearson test found that PCOS patients serum Ghrelin, Orexin-A levels and sex hormones and glucose and lipid metabolism index level has a direct correlation (P <0.05). Conclusion Low levels of Ghrelin and Orexin-A exist in PCOS patients, which can objectively reflect the changes of sex hormones and glycolipid metabolism.
