目的:通过门诊药房信息化药品调剂差错防范体系的建立与运行,有效降低药品调剂差错率,提高药学服务质量,保障患者安全用药。方法:从药品调剂前、中、后3个维度分别展开,运用信息化技术,设计并建立“医师-药师-患者”三位一体的药品调剂差错防范体系,并对该体系运行前后效果进行分析比较。结果:门诊药房信息化药品调剂差错防范体系建立运行后,药品调剂工作的各环节差错均显著减少,调剂前医师开方错误率从0.724 4%下降为0.340 3%;调剂中药师总调配差错率从0.078 8%下降为0.042 4%,其中各调配差错类别的占比也均有所下降;调剂后出门差错率从0.000 68%下降为0.000 26%。结论:门诊药房信息化药品调剂差错防范体系的建立,为减少药品调剂差错提供了有效的新方法,使药师充分发挥专业技术水平,为患者提供优质的信息化药学服务,保障患者安全用药,是未来医院药房降低药品调剂差错率的发展趋势。
OBJECTIVE: To establish and operate the prevention and control system for drug misdiagnosis in outpatient pharmacy, effectively reduce the error rate of drug dispensing, improve the quality of pharmacy service and ensure the safe use of drug in patients. Methods: From the three dimensions before, during, and after the pharmaceutical preparations were respectively carried out, and the information technology was used to design and establish the system of preventing and controlling drug dispensing error of the “physician - pharmacist - patient” trinity and to analyze the effect of the system before and after the operation Compare Results: After the establishment of the system of prevention and control of pharmacy misdiagnosis in outpatient pharmacy, errors in all aspects of drug dispensing were significantly reduced, and the error rate of prescribing was reduced from 0.724 4% to 0.340 3%. Conclusion: The establishment of prevention and control system of drug misdiagnosis in outpatient pharmacy provides an effective new method to reduce drug misdiagnosis, enable pharmacists to give full play to the professional technical level, provide high-quality informative pharmacy services to patients and ensure safe drug use for patients.
