目的 :研究乙酰乙酸 (AcAc)、β -羟丁酸 (β-HBA)检测用于糖尿病 (DM )尤其早期糖尿病诊断的可能性。方法 :以酶法测定AcAc、β-HBA试剂盒、美国BECKMANCX5全自动生化分析仪对空腹血糖 <6 .1mmol/L的正常组 (NFG)组 10 2例、空腹血糖 6 .1~ 6 .9mmol/L的空腹血糖升高组 (IFG组 ) 98例、空腹血糖 >7.0mmol/L(DM组 ) 88例共三组 ,分别测定AcAc、β -HBA、AcAc +β -HBA ,并用u检验。结果 :NFG组与IFG组、DM组三组指标有极显著差异 ,t≥t(n’) 0 .0 1,P≤ 0 .0 1。诊断DM的敏感性 β -HBA +AcAc 90 .1% ,β -HBA81.1% ,AcAc 6 8.7% ;准确性 β -HBA +AcAc 90 .1% ,β -HBA 87.3% ,AcAc 79.3% ;特异性 β -HBA +AcAc 93.3% ,β -HBA 90 .3% ,AcAc 87.5 %。IFG组 :特异性 β -HBA +AcAc 94 .3% ,β -HBA 90 .3% ,AcAc93.4 % ;准确性 :β -HBA +AcAc 78.5 % ,β -HBA 70 .8% ,AcAc 70 .8% ;敏感性 :β -HBA +AcAc 6 7.4 % ,β - HBA 6 1.6 % ,AcAc 6 0 .6 %。由统计量建立的血糖与β -HBA +AcAc回归方程为 y =1.76 +10 .75x ,相关系数r=0 .6 2 1。结论 :β -HBA +AcAc比单用 β -HBA或AcAc诊断糖尿病优越 ,与血糖测定并用 ,可试用于诊断糖尿病
Objective: To investigate the possibility of using AcAc and β - HBA in the diagnosis of diabetes mellitus (DM) especially in early stage. Methods: Enzyme-linked immunosorbent assay (ELISA) was used to detect AcAc, β-HBA kit and BECKMANCX5 automatic biochemical analyzer in 102 cases of fasting plasma glucose <6.1mmol / L in normal group (NFG) 98 cases of fasting plasma glucose (IFG group) / 88 cases of fasting plasma glucose> 7.0mmol / L (DM group) were divided into three groups. AcAc, β -HBA and AcAc + β -HBA were measured respectively. Results: The indexes of three groups of NFG group, IFG group and DM group had extremely significant difference, t≥t (n ’) 0. 01, P≤0. Diagnostic sensitivity of DM β -HBA + AcAc 90.1%, β -HBA81.1%, AcAc 6 8.7%; Accuracy β -HBA + AcAc 90.1%, β -HBA 87.3%, AcAc 79.3% 93.3% of β -HBA + AcAc, 90.3% of β -HBA, and 87.5% of AcAc. IFG group: specific β -HBA + AcAc 94.3%, β -HBA 90.3%, AcAc 93.4%; accuracy: β -HBA + AcAc 78.5%, β -HBA 70.8%, AcAc 70. Sensitivity: 7.4% of β -HBA + AcAc 6, 1.6% of β - HBA 6, and 0.6% of AcAc. The regression equation of blood glucose and β -HBA + AcAc established by statistics was y = 1.76 +10 .75x, and the correlation coefficient was r = 0.62. Conclusion: β -HBA + AcAc is superior to β-HBO or AcAc alone in the diagnosis of diabetes. It can be used in the diagnosis of diabetes mellitus
