目的 :了解用海脉素行急性等容血液稀释后血浆渗透压的变化 ,以探讨其对颅脑肿瘤患者术中、术后脑水肿的影响。方法 :选择颅脑肿瘤手术病人 12例 ,麻醉后从桡动脉放血 8ml.kg-1 ,同时经静脉等速输入等量的海脉素。测定血液稀释前后的钾、钠、氯、钙离子浓度、尿素氮及血糖浓度、比较血液稀释前后的血浆渗透压。结果 :与血液稀释前比较 ,血浆渗透压无显著性变化 (P〉0 .0 5) ,血糖有所升高 (P〈0 .0 5)但在正常值范围。结论 :轻度等容血液稀释对血浆渗透压的影响不大 ,不增加颅脑肿瘤病人脑水肿的机会 ,是一种有效的血液保护方法。
Objective: To understand the changes of plasma osmolality after acute isovolemic hemodilution with Salvia miltiorrhiza to explore its effect on intracranial and postoperative cerebral edema in patients with brain tumor. Methods: Twelve patients with craniocerebral tumor surgery were selected, and 8ml.kg-1 of radial blood was taken from the radial artery after anesthesia. At the same time, the same amount of melanin was input through vein. Before and after hemodilution, the concentrations of potassium, sodium, chloride, calcium, urea nitrogen and blood glucose were measured to compare the plasma osmotic pressure before and after hemodilution. Results: There was no significant change in plasma osmolality (P> 0.05) and blood glucose (P <0.05) but in the normal range before hemodilution. Conclusions: Mild blood plasma has little effect on plasma osmotic pressure and does not increase the chance of cerebral edema in patients with brain tumor. It is an effective blood protection method.
