庆大霉素主要是对第八对颅神经和肾脏有毒性作用。我科曾遇到三例因大剂量应用该药导致精神症状的病例。报道如下。例1,女,14岁。1978年10月2日以左手拇指刀伤八日左上肢肿痛四日入院。伴发冷发烧。体检:体温39℃,脉搏124次/分,呼吸30次/分,血压80/60毫米汞柱,体重30公斤。热病容,神志清楚,全身皮肤散在小脓点。左上肢肿胀,皮肤潮红,左手拇指桡侧有一2cm长斜行感染伤口。心肺(-)。白细胞计数51400/mm~3,中性92％。诊断:左手拇指外伤感染;左上肢蜂窝织炎;败血症。投以青、链霉素,局部行外科处理。10天后换用庆大霉素8万单位,每8小时肌注一次,用药8天后出院幻视、狂躁、哭闹、失眠、分析与庆大霉素有关。即又改用青、链霉素,3日后上述症状消失。例2:女,51 Gentamicin is primarily toxic to the eighth cranial nerve and to the kidneys. Vote green, streptomycin, local line surgical treatment. 10 days after the exchange of gentamicin 80,000 units, intramuscular injection once every 8 hours, 8 days after treatment, discharge of visual hallucinations, manic, crying, insomnia, analysis and gentamicin.
