目的观察三腔二囊管在治疗食管静脉曲张破裂大出血中的价值。方法对2008年1月至2011年6月我院消化内科住院治疗的218例食管静脉曲张破裂大出血患者的临床资料进行整理和分析,根据患者及患者家属的意愿分为气囊压迫止血组和药物治疗组,气囊压迫止血组食管静脉曲张破裂大出血患者在使用奥曲肽进行治疗的基础上给予三腔二囊管压迫止血,药物治疗组仅进行奥曲肽治疗,观察24h,比较两组治疗的临床疗效。结果气囊压迫止血组91例患者有效,有效率为83.49%,而药物治疗组78例有效,有效率为71.56%,经χ2检验两组治疗有效率差异有统计学意义(χ2=4.449,P=0.035)。结论三腔二囊管联合奥曲肽疗效优于单纯药物治疗,是治疗食管静脉曲张破裂大出血安全有效的治疗方案,可进行临床推广运用。
Objective To observe the value of three-chamber two-tube in the treatment of esophageal variceal hemorrhage. Methods The clinical data of 218 patients with esophageal variceal bleeding in our hospital from January 2008 to June 2011 were collected and analyzed. According to the wishes of the patients and their families, the patients were divided into balloon compression hemostasis group and drug treatment Group, balloon compression hemostasis patients with esophageal variceal bleeding in the use of octreotide for treatment based on the three-chamber two-capsule tube to stop bleeding, the drug treatment group only octreotide treatment 24h observation, the clinical efficacy of the two groups were compared. Results 91 patients with balloon compression hemostasis were effective and effective rate was 83.49%, while 78 cases were effective in treatment group, the effective rate was 71.56%. There was significant difference between the two groups (χ2 = 4.449, P = 0.035). Conclusions The efficacy of triple-lumen capillaries combined with octreotide is better than that of pure drug therapy. It is a safe and effective treatment for esophageal variceal bleeding and can be used clinically.
