目的了解医院肺癌住院患者真菌感染的状况,分析相关的危险因素,为控制肺癌患者的真菌感染提供理论依据。方法回顾性调查分析医院肿瘤科2010年1月-2013年12月857例肺癌患者的临床资料;计算每年真菌感染率及平均感染率,对肺癌住院患者发生真菌感染的相关因素进行分析;采用Excel2007对数据进行整理,SPSS17.0进行数据分析。结果 857例肺癌患者中发生真菌感染135例,感染率为15.8%;2010-2013年肺癌患者感染率分别为13.2%、15.4%、16.4%和17.5%,呈逐年上升趋势;年龄≥65岁、有吸烟史、有基础疾病、肺癌中晚期、住院时间≥14d、有侵入性操作、预防性使用抗菌药物、进行放(化)疗治疗、手术治疗是肺癌患者真菌感染的相关影响因素(P<0.05)。结论肺癌住院患者真菌感染率较高,且呈逐年上升趋势,肺癌患者发生真菌感染的危险因素较为复杂,建议从积极治疗基础疾病、严格控制各种侵入性检查、避免预防性使用抗菌药物、早期治疗缩短住院时间等方面进行预防和控制,降低真菌感染率。
Objective To understand the status of fungal infection in hospitalized patients with lung cancer and to analyze the related risk factors so as to provide a theoretical basis for controlling the fungal infection in lung cancer patients. Methods The clinical data of 857 lung cancer patients from January 2010 to December 2013 in our hospital were analyzed retrospectively. The annual fungal infection rate and average infection rate were calculated. The related factors of fungal infection in hospitalized patients with lung cancer were analyzed. Sort data, SPSS17.0 for data analysis. Results 135 cases of fungal infections were found in 857 lung cancer patients, the infection rate was 15.8%. The infection rates of lung cancer patients in 2010-2013 were 13.2%, 15.4%, 16.4% and 17.5% respectively, showing an increasing trend year by year. There are smoking history, basic diseases, advanced stage of lung cancer, hospitalization time≥14 days, invasive operation, preventive use of antibacterial drugs, radiotherapy and chemotherapy, and surgical treatment are the related factors of fungal infection in patients with lung cancer (P < 0.05). Conclusion The prevalence of fungal infection in hospitalized patients with lung cancer is high and the trend is increasing year by year. The risk factors of fungal infection in patients with lung cancer are more complicated. It is suggested that the active treatment of underlying diseases should be strictly controlled and various invasive tests should be strictly controlled to prevent the prophylactic use of antibiotics. Treatment and shorten hospital stay, prevention and control, reduce the rate of fungal infection.
