[目的]探讨噬菌体生物扩增法检测结核杆菌在糖尿病合并结核感染快速诊断中的应用价值。[方法]收集本院结核病区及门诊糖尿病患者的痰、支气管镜镜抽取液标本共89份,应用噬菌体生物扩增法检测结核分支杆菌,同时作抗酸染色、并用VersaTREKTM细菌培养仪做分支杆菌培养。噬菌体生物扩增法和/或培养阳性的菌株32株做分支杆菌菌种鉴定。[结果]噬菌体生物扩增法检测的阳性率31.4%(28/89)显著高于涂片法19.1%(17/89)(P﹤0.01),与VersaTREKTM培养法阳性率28.1%(25/89)相比,差异无统计学意义(P﹥0.05)。21株VersaTREKTM培养法与噬菌体生物扩增法同时阳性的菌株和7株噬菌体生物扩增法阳性菌株均鉴定为结核分支杆菌;4株VersaTREKTM培养法阳性菌株鉴定为非结核分支杆菌。[结论]噬菌体生物扩增法检测结核分支杆菌快速、特异、敏感,在糖尿病合并结核感染的快速诊断中有重要价值。
[Objective] The purpose of this study was to explore the value of bacteriophage bioassay in the detection of Mycobacterium tuberculosis in the diagnosis of diabetes complicated with tuberculosis. [Methods] A total of 89 specimens of sputum and bronchoscopic samples were collected from TB patients and outpatients with diabetes mellitus. Mycobacterium tuberculosis was detected by phage amplification. At the same time, acid-fast staining was performed. Mycobacteria were cultured with VersaTREKTM bacterial culture instrument to cultivate. Thirty-two phage biological amplification methods and / or culture-positive strains were identified as mycobacterial species. [Results] The positive rate of phage amplification assay was 31.4% (28/89), significantly higher than that of smear method (19.1%, 17/89) (P <0.01), and the positive rate of VersaTREKTM culture method was 28.1% (25/89 ), The difference was not statistically significant (P> 0.05). Twenty-one strains of VersaTREKTM were identified as Mycobacterium tuberculosis simultaneously with the phage amplification method and seven strains of phage amplified by biopanning method. Four strains positive for VersaTREKTM culture were identified as Mycobacterium tuberculosis. [Conclusion] The detection of Mycobacterium tuberculosis by phage biological amplification method is rapid, specific and sensitive, and it is of great value in the rapid diagnosis of diabetic combined with tuberculosis infection.
