目的探讨三维超声造影增强显像(3D CEUS)技术在消化系统肿瘤诊断中的应用价值。方法经手术病理或CT印证的消化系统肿瘤81例,分成良性病灶组和恶性病灶组。静脉注射超声造影剂SonoVue,应用低机械指数造影方法,在二维超声造影增强显像(2D CEUS)的基础上进行3D CEUS。描记二维造影的时间-强度曲线,将三维造影显像特征进行评分,计算3D CEUS对恶性病灶诊断的敏感性、特异性和预期准确性。结果 3D CEUS可显示病灶整体的立体影像、结构层次和血管空间分布,较2D CEUS更为形象,并可从不同角度观察恶性肿瘤的血管特征。应用3D CEUS特征评分,得出对本组消化系统恶性病灶诊断的敏感性、特异性和预期准确性分别为97.7%、94.7%和95.5%。结论 3D CEUS使超声图像的显像质量得到新的提高,为消化系统肿瘤的临床诊断提供了更多的补充信息。
Objective To investigate the value of three-dimensional contrast enhanced imaging (3D CEUS) in the diagnosis of digestive system tumors. Methods 81 cases of digestive system tumors confirmed by pathology or CT were divided into benign lesion group and malignant lesion group. SonoVue, an intravenous contrast agent, was used to perform 3D CEUS based on two-dimensional CEUS with low-resolution mechanical imaging. The time-intensity curve of two-dimensional angiography was traced, and the three-dimensional contrast imaging characteristics were scored. The sensitivity, specificity and expected accuracy of 3D CEUS in the diagnosis of malignant lesions were calculated. Results 3D CEUS can display the whole three-dimensional image of the lesion, the level of the structure and the spatial distribution of the blood vessels. It is more image than 2D CEUS and can observe the vascular features of malignant tumors from different angles. Using the 3D CEUS feature score, the sensitivity, specificity and expected accuracy of the diagnosis of malignant lesions of the digestive system were 97.7%, 94.7% and 95.5% respectively. Conclusions 3D CEUS can improve the imaging quality of ultrasound images and provide more supplementary information for the clinical diagnosis of digestive system tumors.
