患儿,男,8岁,既往无过敏史.因体检发现乙肝入院.入院体检:黄疸(一).心肺无异常,肝右肋下0.5cm,脾未触及,腹水征(一),无浮肿,未见肝掌蜘蛛痣,浅表淋巴结不肿大.入院诊断;慢性迁延型乙型病毒性肝炎,入院后肌注猪苓多糖40mg(江苏连云港东风制药厂,生产批号930412—2).10min后患儿出现全身皮肤潮红(无搔痒)、咳嗽、声嘶、眼睑及上下嘴唇水肿,睑结膜明显充血.P98次/min、律齐,无杂音.立即肌注异丙嗪25mg,静脉点滴10%葡萄糖200mL十地塞米松5mg.约半小时后患儿症状逐渐缓解,次日停用猪苓多糖、改用其它药未见不适.第3日再次肌注猪等多糖20mg.不到10min 患儿即出现前述症状,如前治疗后症状缓解此后停用猪苓多糖观察1周未见上述反应.
