目的分析儿童外伤诱发过敏性紫癜(HSP)10例的临床表现及治疗预后。方法对外伤诱发HSP患儿10例临床资料进行回顾性分析。结果外伤诱发HSP患儿均在入院前后出现皮疹,以双下肢最多见,皮疹多对称分布,略高于皮面、压之不褪,部分融合成片。8例伴关节肿痛,7例伴消化道症状,1例伴肾脏损害。血清IgA水平升高,T淋巴细胞群紊乱。常规治疗基础上对合并消化道症状患儿给予小剂量地塞米松及钙剂、抑酸等治疗。3例出院后有轻度皮疹反复。结论外伤为HSP的诱发因素之一,在合并消化道症状、关节肿痛的患儿可酌情使用小剂量糖皮质激素。
Objective To analyze the clinical manifestations and prognosis of 10 children with Henoch-Schonlein Purpura (HSP) after traumatic injury. Methods The clinical data of 10 children with HSP induced trauma were retrospectively analyzed. Results The children with traumatic stress-induced HSP had rash both before and after admission. Most of the patients had bilateral lower extremities. Symmetrical rash distribution was found. 8 cases of joint pain, 7 cases with gastrointestinal symptoms, 1 case with renal damage. Serum IgA levels, T lymphocyte disorders. On the basis of routine treatment of children with gastrointestinal symptoms combined with small doses of dexamethasone and calcium, acid suppression and other treatment. Three patients had mild rash repeatedly after discharge. Conclusion Traumatic injury is one of the inducing factors of HSP. Children with gastrointestinal symptoms and joint swelling and pain may use low-dose glucocorticoids as appropriate.
