目的研究磷脂酰肌醇3-激酶(phosphoinositide 3-kinase,PI3K)和蛋白激酶B(protein kinase B,Akt)在人喉鳞状细胞癌组织中的表达特征及临床意义。方法应用免疫组化S-P法检测40例喉鳞癌、20例声带息肉和20例喉乳头状瘤组织中PI3K和p-Akt的蛋白表达,显微镜下计数阳性细胞,对结果进行统计学分析。结果PI3K和p-Akt在声带息肉组织中均呈低表达;在喉乳头状瘤中,PI3K高表达,而p-Akt呈低表达;在喉鳞状细胞癌组织中,PI3K和p-Akt均呈高表达。喉鳞状细胞癌中PI3K和p-Akt的表达与喉癌临床分期、颈部淋巴结转移及肿瘤的原发部位均无明显差异(P>0.05)。PI3K和p-Akt蛋白的阳性表达率随喉鳞癌分化程度的降低而降低,差异有统计学意义(P<0.01)。同一标本中PI3K和p-Akt的表达有显著的相关性(r=0.687,P<0.01)。结论喉鳞癌存在PI3K和Akt蛋白的异常表达并与癌细胞的病理分级关系密切,PI3K/Akt途径的异常活化可能在喉鳞状细胞癌发生过程中起重要作用。
Objective To investigate the expression and clinical significance of phosphoinositide 3-kinase (PI3K) and protein kinase B (Akt) in human laryngeal squamous cell carcinoma (HCC). Methods The protein expressions of PI3K and p-Akt in 40 cases of laryngeal squamous cell carcinoma, 20 cases of vocal cord polyps and 20 cases of laryngeal papilloma were detected by immunohistochemical S-P method. The positive cells were counted under microscope, and the results were statistically analyzed. Results The expression of PI3K and p-Akt was low in vocal cord polyps, while in laryngeal papilloma, the expression of PI3K was high and the expression of p-Akt was low. In laryngeal squamous cell carcinoma, both PI3K and p-Akt High expression. The expression of PI3K and p-Akt in laryngeal squamous cell carcinoma had no significant difference with clinical stage, cervical lymph node metastasis and primary tumor site (P> 0.05). The positive expression rate of PI3K and p-Akt protein decreased with the degree of differentiation of laryngeal squamous cell carcinoma, the difference was statistically significant (P <0.01). There was a significant correlation between the expression of PI3K and p-Akt in the same specimen (r = 0.687, P <0.01). Conclusions The abnormal expression of PI3K and Akt protein in laryngeal squamous cell carcinoma is closely related to the pathological grade of the cancer cells. The abnormal activation of PI3K / Akt pathway may play an important role in the carcinogenesis of laryngeal squamous cell carcinoma.
