目的 应用经颧弓改良翼点入路显微手术,切除巨大蝶骨嵴脑膜瘤。方法 改良翼点入路,主要为切断颧弓,尽量暴露中颅底,在显微镜下操作,分块切除肿瘤,避免损伤颈内动脉等重要结构。结果 13例巨大蝶骨 嵴脑膜瘤在显微镜下做到了全切,术后出现脑血管痉挛4例,精神症状6例及皮下积液8例等。给予相应处理后, 均在短期内恢复。结论 经颧弓改良翼点入路显微手术,能够充分暴露中颅底,肿瘤显露良好,镜下视野清楚,避免 损伤了重要结构,特别有利于切除内侧型肿瘤。
The purpose of application zygomatic arch improved pterional approach microsurgery, removal of giant sphenoid ridge meningioma. Methods Improved pterional approach, mainly to cut off the zygomatic arch, try to expose the middle cranial base, under the microscope operation, block removal of the tumor, to avoid damage to the internal carotid artery and other important structures. Results Thirteen patients with giant sphenoid ridge meningioma underwent total resection under microscope. There were 4 cases of cerebral vasospasm, 6 cases of psychosis and 8 cases of subcutaneous effusion. Given the appropriate treatment, are resumed in the short term. Conclusion Microsurgical treatment of pterional zygomatic pterygia with pterional approach can fully expose the mid-skull base. The tumors are well exposed and the microscopic field of vision is clear. Important structures can be avoided and the resection of medial tumors is particularly beneficial.
