目的通过总结卵巢良、恶性肿瘤的多普勒超声表现,结合病理诊断,探讨频谱多普勒超声技术对区分卵巢肿瘤良、恶性的诊断价值。方法 2010年1月—2013年12月于该院就诊,临床怀疑有卵巢包块的女性患者245例。术前进行频谱多普勒超声检查,记录肿物的频谱多普勒参数(包括阻力指数和搏动指数)。术后对肿物病理诊断跟踪随访,对上述结果进行统计学分析。结果 245例肿物进行病理诊断后,共诊断出良性肿瘤160例,恶性肿瘤85例,按照RI值<0.55或PI值<0.75(恶性组珚x±s)作为多普勒超声检查诊断恶性肿瘤的依据,频谱多普勒检查诊断恶性肿瘤的灵敏度=89.41%,特异度=77.50%。结论卵巢良、恶性肿瘤在阻力指数、搏动指数等方面存在差异,多普勒超声成像技术能够根据卵巢肿瘤的血流的特点在一定程度上进行定量分析,对卵巢原发性恶性肿瘤的早期诊断具有一定的价值。
OBJECTIVE: To evaluate the diagnostic value of Doppler ultrasonography in differentiating benign and malignant ovarian tumors by summarizing the Doppler echocardiography of benign and malignant ovarian tumors and combining with pathological diagnosis. Methods From January 2010 to December 2013, 245 female patients with clinically suspected ovarian masses were treated in this hospital. Preoperative spectral Doppler ultrasound examination, recording the tumor spectrum Doppler parameters (including the resistance index and pulsatility index). Postoperative follow-up of the pathological diagnosis of the tumor, the above results were statistically analyzed. Results A total of 245 malignant tumors were diagnosed in 160 cases of benign tumors and 85 of malignant tumors by pathological diagnosis. According to Doppler ultrasonography, RI value <0.55 or PI value <0.75 (malignant group x ± s) The basis of spectral Doppler detection of malignant tumor sensitivity = 89.41%, specificity = 77.50%. Conclusions There are differences in resistance index and pulsatility index between benign and malignant ovarian tumors. Doppler ultrasound imaging can be quantitatively analyzed according to the characteristics of blood flow in ovarian tumor, and the early diagnosis of ovarian primary malignant tumor Has a certain value.
