患者女,62岁。因患结肠癌行左半结肠切除术,术后半年按医嘱服用福铁隆,每次200mg,一日3次。服用8d后,常规体检发现谷草转氨酶增至84u/L,乳酸脱氢酶增至297u/L,心型肌酸激酶增至286u/L,肌酸激酶增至2929u/L,其中肌酸激酶为最高(正常值为0～173u/L)。患者自述身体未感不适,无既往心脏疾病史,近日无上呼吸道、消化道感染病史,并未服用其它药物,除心肌酶谱外,经门诊检查其它项目均为正常。试停用福铁隆,心肌酶谱逐呈梯度下降。6d后,谷草转氨酶降至65.02u/L,乳酸脱氢酶降至288u/L,心型肌酸肌酶降至61.35u/L,肌酸激酶降至1233u/L。10d后,谷草转氨酶及乳酸脱氢酶降至正常,心型肌酸激酶降至27u/L,肌酸激酶降至382u/L。停药15d后,各种酶均恢复正常值。随访半年,无心肌酶增高现象。 讨论:福铁隆(Furtulon)是日本罗西欧股份有限公司生产的胶囊剂,常见的不良反应有腹泻、白细胞下降、食欲不振、恶心、呕吐、血小板减少、皮肤瘙痒;少见的不良反应有心电图异常、锥体外系反应等。心肌酶谱增高现象未见报道。因患者近日无上呼吸道、消化道感染等其它可致心肌酶增高之诱因,停药后心肌酶谱即逐渐恢复正常,且停药半年后也未出现上述现象,故笔者认为是药物所致。由此考虑,福铁隆可使心肌细胞受损,导致药物中毒性心肌炎 Female patient, 62 years old.
