目的:探讨不同种类心脏病在妊娠合并心脏病中的发病率,分析不同心功能状态对孕产妇和围产儿的影响。方法:对2004年1月~2008年12月郑州大学第一附属医院73例妊娠合并心脏病患者的临床资料进行回顾性分析。结果:妊娠合并心脏病以先天性心脏病发病率最高(33例占45.21%),其次为风湿性心脏病(22例占30.14%),不明原因心律失常(14例占19.18%),高血压性心脏病(3例占4.11%),心肌炎(1例占1.37%)。心功能Ⅰ~Ⅱ级与Ⅲ~Ⅳ级比较,妊娠孕周、新生儿平均体重差异有统计学意义(P<0.05)。73例妊娠合并心脏病,剖宫产率90.41%,围产儿死亡率为0。结论:先天性心脏病在妊娠合并心脏病发病率中居首位,而在农村边远地区,风湿性心脏病仍占相当大的比例。不同心功能状态影响孕周及新生儿体重,适时剖宫产术可改善妊娠结局。
Objective: To investigate the incidence of different types of heart disease in pregnancy complicated with heart disease and to analyze the influence of different cardiac function on pregnant women and perinatals. Methods: The clinical data of 73 pregnant women with heart disease complicated with heart disease admitted to the First Affiliated Hospital of Zhengzhou University from January 2004 to December 2008 were analyzed retrospectively. Results: The incidence of congenital heart disease in pregnant women with the highest incidence of congenital heart disease (33 cases accounted for 45.21%), followed by rheumatic heart disease (22 cases accounted for 30.14%), unexplained arrhythmia (14 cases accounted for 19.18%), hypertension Cardiac heart disease (3 cases accounted for 4.11%), myocarditis (1 case accounted for 1.37%). Heart function Ⅰ ~ Ⅱ level and Ⅲ ~ Ⅳ level compared gestational weeks, the average weight of newborns was statistically significant (P <0.05). 73 cases of pregnancy complicated by heart disease, cesarean section rate of 90.41%, perinatal mortality rate of 0. CONCLUSIONS: Congenital heart disease ranks first in the incidence of pregnancy-associated heart disease, while in remote rural areas, rheumatic heart disease still accounts for a significant proportion. Different heart function status affect gestational age and newborn weight, timely cesarean section can improve pregnancy outcomes.
