探讨CK19、Galectin-3和TPO在甲状腺乳头状癌(Papillary thyroid carcinoma,PTC)与恶性潜能未定的高分化肿瘤(Well-differentiated tumor of uncertain malignant potential,WDTUMP)中的表达及鉴别诊断意义.用免疫组化方法检测50例PTC、71例WDT-UMP和35例正常甲状腺组织(对照组)中CK19、Galectin-3、TPO的表达情况.结果显示:PTC中,CK19、Galectin-3和TPO的阳性率分别为100%、94%和12%,CK19和Galectin-3以弥漫强阳性表达为主,TPO以阴性表达为主;WDT-UMP中,CK19、Galectin-3和TPO的阳性率分别为54.9%、69%和88.7%,CK19和Galectin-3以弱阳性或阴性为主,TPO呈弱阳性至强阳性表达.正常组织中,CK19、Galectin-3和TPO的阳性率分别为0、2.9%和97.1%,CK19和Galectin-3基本不表达,TPO以强阳性表达为主.CK19、Galectin-3在PTC中的表达均高于WDT-UMP及正常甲状腺组织,TPO在PTC中的表达均低于其余两组.联合应用CK19和TPO鉴别PTC与WDTUMP时,可获得较高的敏感性、特异性、诊断准确率,分别为88%、91.3%、89.9%.在WDTUMP中,CK19与Galectin-3的表达正相关(P<0.05),CK19、Galectin-3与TPO的表达负相关(P<0.05);在PTC中,CK19与Galectin-3的表达正相关(P<0.05),与TPO的表达负相关(P<0.05).因此,CK19、Galectin-3、TPO可以作为鉴别PTC及WDT-UMP的重要参考标记,三种抗体联合应用可提高鉴别PTC及WDT-UMP的诊断准确率.
