患者武××,男,37岁,因胃脘隐隐作痛,呕吐清水,从来畏用冷饮冷食。每发作时则胃痛喜按,得暖可缓,偶下黑便等。诊为虚寒腹痛,虽经中西医多方治疗,见效甚微。于89年5月某医院胃镜检查,提示溃疡病。据此投以加味归芪建中汤: 当归12克、黄芪30克、桂枝10克、白芍15克、百合30克、乌药9克、元胡10克、川楝子10克、白及10克、蜂蜜2茶匙,煎服。连投3剂,虽胃痛稍缓,但仍反复发作,有日益加重趋势。乃以前方加黑附子10克、干姜6克,又三剂,胃痛大减,效未更方,继投10余剂,一切症状消失,再未复发。考虑白及甘寒,性粘,可保护溃疡面,附子性热逐寒。但十八反有“半萎贝蔹及攻乌”之戒,一寒一热、一敛一散,为两仇不共;又见前贤示“相反亦可相成”之说,衡量利弊,认为此方无主药可比,遂用白及、附子常量,加之甘草、蜂蜜可缓众药之毒,料以无妨。试投之后,果然诸症俱失。可见反药用之得当,能收桴鼓之效。
