目的探讨维持性血液透析炎症因子水平与患者心功能及营养状态的关系。方法采用酶联免疫吸附法检测患者血液中IL-6、TNF-α水平,探讨血液透析患者炎症状态与患者心功能及营养状态的关系。结果 TNF-α高水平组患者左心房内径、左心室舒张末期内径、射血分数、B型脑钠肽等各指标水平均明显差于TNF-α低水平组患者;IL-6高水平组患者以上指标均明显差于IL-6低水平组患者,上述差异均有统计学意义(P<0.05)。TNF-α高水平组患者轻度营养不良占20%、中度营养不良占60%,重度营养不良占20%,明显差于TNF-α低水平患者;IL-6高水平组各营养指标均差于IL-6低水平组患者,TNF-a、IL-6水平与LAD、LVEDd、MIS、BNP评分成正相关,P<0.05;与EF成负相关,P<0.05。结论血液透析患者炎症因子水平与患者心功能及营养状态密切相关。
Objective To investigate the relationship between the level of maintenance hemodialysis inflammatory cytokines and cardiac function and nutritional status in patients. Methods The levels of IL-6 and TNF-α in blood of patients were detected by enzyme-linked immunosorbent assay (ELISA), and the relationship between inflammatory status and cardiac function and nutritional status of hemodialysis patients was explored. Results The level of left atrium, left ventricular end-diastolic diameter, ejection fraction and B-type natriuretic peptide in patients with high TNF-α level were significantly lower than those in patients with low level of TNF-α. Patients with high level of IL-6 The above indexes were significantly lower than those in the low level of IL-6, the above differences were statistically significant (P <0.05). Patients with high levels of TNF-α accounted for 20% of mild malnutrition, moderate malnutrition accounted for 60%, severe malnutrition accounted for 20%, significantly worse than those with low levels of TNF-α; The levels of TNF-a and IL-6 were positively correlated with the score of LAD, LVEDd, MIS and BNP in patients with low IL-6 level (P <0.05), but negatively correlated with EF (P <0.05). Conclusion The level of inflammatory cytokines in patients with hemodialysis is closely related to the cardiac function and nutritional status.
