目的评价妊娠晚期高龄孕妇胎儿在宫内的整体生长发育情况,从而提高高龄产妇的分娩质量。方法回顾性分析2014年3月-2016年3月经该院超声科行超声检查的95例妊娠晚期高龄孕妇(高龄组)的超声检查结果,对比分析40例同期适龄孕妇(适龄组)的超声检查结果,对其畸形补漏筛查情况、胎儿生长发育以及彩色多普超声检查结果作相关的统计学分析。结果适龄组与高龄组胎儿房间隔缺损比较,差异有统计学意义(χ~2=8.239,P<0.05),而I度唇腭裂两组比较,差异无统计学意义(χ~2=0.868,P>0.05);与适龄组相比,高龄组经产胎儿的头围(HC)较小(t=12.11,P<0.05),腹围(AC)较小(t=3.05,P<0.05),股骨长(FL)度较短(t=3.68,P<0.05),而适龄组与高龄组初产胎儿头围,腹围及股骨长度比较,差异均无统计学意义(均P>0.05);与适龄组胎儿相比,高龄组胎儿脐动脉阻力指数(RI)、脐动脉搏动指数(PI)及收缩期与舒张期血流速度之比(S/D)值显著增高,差异有统计学意义(均P<0.05);与适龄组胎儿心脏血流动力学相比,仅高龄组经产胎儿的肺动脉峰值加速时间及射血时间差异有统计学意义(均P<0.05)。结论高龄妊娠晚期胎儿生长发育可能迟缓,胎儿的宫内风险增加,胎儿房间隔缺损的发生率增高。加强超声检查、降低生育年龄,有望提高分娩及胎儿质量。
Objective To evaluate the overall intrauterine growth and development of pregnant women in the third trimester of pregnancy, so as to improve the quality of delivery of the elderly maternal. Methods We retrospectively analyzed the ultrasonographic results of 95 pregnant women with advanced pregnancy (elder group) who underwent ultrasonography in our hospital from March 2014 to March 2016 and compared the ultrasound findings of 40 pregnant women of the same period (age group) The results of its deformity detection of leak detection, fetal growth and color Doppler ultrasound findings for the relevant statistical analysis. Results There was significant difference in age-matched fetuses with atrial septal defect between the age group and the advanced age group (χ ~ 2 = 8.239, P <0.05), while there was no significant difference between the two groups (χ ~ 2 = 0.868, (T = 12.11, P <0.05), abdominal circumference (AC) was smaller (t = 3.05, P <0.05) , Femur length (FL) was shorter (t = 3.68, P <0.05), while no significant difference was found in the head circumference, abdominal circumference and femur length of the primiparous fetus between the age group and the advanced age group (all P> 0.05) (RI), pulsatility index (PI) and systolic and diastolic blood flow velocity ratio (S / D) of fetus in senior age group were significantly higher than those of age group, the difference was statistically significant (P <0.05). Compared with the fetal heart hemodynamics in the age group, there was significant difference in peak pulmonary artery acceleration time and ejection time among the fetuses in the advanced age group (all P <0.05). CONCLUSION: Fetal growth and development may be slow in advanced pregnancy, the intrauterine risk of fetus increases, and the incidence of fetal atrial septal defect increases. Strengthen the ultrasound examination, reduce the childbearing age, is expected to improve childbirth and fetal quality.
