目的:评价前列腺段尿道提拉术(prostatic urethral lift,PUL)在治疗BPH引起的下尿路症状(lower urinary tract symptom,LUTS)和性功能改善方面的研究。方法:检索PubMed、Cochrane Library、Web of science及ScienceDirect数据库关于PUL治疗由BPH引起的LUTS和性功能改善方面的文献资料,并依据Cochrane操作员手册进行文献筛选、提取资料数据及质量评价。应用RevMan 5.2版本的软件进行数据处理分析。结果:系统纳入1篇随机对照实验,5篇非随机对照实验,共290例患者。评估了PUL术后12个月国际前列腺症状评分(IPSS评分)降低10.63(95%CI:-11.66~-9.60),最大尿流率(Qmax)升高3.43(95%CI:2.71~4.14),男性性健康问卷-射精功能障碍(MSHQ-EjD)升高0.93(95%CI:0.22~1.65),男性性健康问卷-困扰评分(MSHQ-Bother)降低0.68(95%CI:-1.03~-0.33),差异有统计学意义(P<0.05)。结论:PUL是一种治疗BPH的微创手术,在缓解下尿路症状、改善男性功能方面具有良好的效果。
OBJECTIVE: To evaluate the efficacy of prostatic urethral lift (PUL) in the treatment of BPH-induced lower urinary tract symptoms (LUTS) and sexual function improvement. Methods: We searched PubMed, Cochrane Library, Web of Science and ScienceDirect database for the literature on PUL for the treatment of BPH-induced LUTS and sexual function improvement. We searched the literature based on the Cochrane Handbook and extracted data and quality evaluation. Apply RevMan 5.2 software for data processing and analysis. Results: One randomized controlled trial and five non-randomized controlled trials were included. A total of 290 patients were included. The International Prostate Symptom Score (IPSS) score was reduced by 10.63 (95% CI: -11.66 to -9.60) and 3.43 (95% CI: 2.71 to 4.14) for the maximum flow rate (Qmax) at 12 months after PUL, Men’s Sexual Health Questionnaire (MSHQ-EjD) increased by 0.93 (95% CI: 0.22-1.65) and decreased by 0.68 (95% CI: -1.03-0.33) for the Men’s Health Questionnaire-MSHQ-Bother ), The difference was statistically significant (P <0.05). Conclusion: PUL is a minimally invasive surgical treatment of BPH and has good effects in alleviating lower urinary tract symptoms and improving male function.
