目的观察继发性胰腺损伤与全身炎症反应综合征(SIRS)患者近期预后的关系。方法观察2007年至2010年48例ICU收治的SIRS患者,均经临床表现、腹部CT检查排除急性胰腺炎、原发性胰腺疾病及胰腺损伤,根据其血清淀粉酶是否升高,分为继发性胰腺损伤组(23例)和对照组(25例),比较2组患者近期预后不良事件(即低血压状态/休克,心、肺、肝、肾、消化道等脏器中1个器官功能损害,多脏器功能障碍及死亡)发生率的差异。结果 48例SIRS患者中,继发性胰腺损伤组近期预后不良事件发生率明显高于对照组(65.2%vs36.0%,P<0.05)。结论继发性胰腺损伤提示SIRS患者近期预后不良,可作为评估SIRS患者住院期间病情危重程度及近期预后的指标之一。
Objective To investigate the relationship between secondary pancreatic injury and systemic prognosis in patients with systemic inflammatory response syndrome (SIRS). Methods From 2007 to 2010, 48 patients with SIRS admitted to the ICU were excluded from acute pancreatitis, primary pancreatic disease and pancreatic injury by clinical manifestations and abdominal CT examination. According to whether serum amylase was elevated, they were divided into secondary (N = 23) and control group (n = 25). The recent adverse events (ie, hypotension / shock, one organ function in heart, lung, liver, kidney, Damage, multiple organ dysfunction and death). Results Among the 48 patients with SIRS, the incidence of adverse events in the patients with secondary pancreatic trauma was significantly higher than that in the control group (65.2% vs36.0%, P <0.05). Conclusions Secondary pancreatic injury suggests that SIRS patients have poor prognosis in the near future, which may be used as an index to assess the severity of illness and the prognosis of SIRS during hospitalization.
