本文报告我院1982年～1992年收治Ⅳ期胃癌并行全胃切除术共44例。消化道重建方式采用Roux-en-Y术式,其中食道空肠端端吻合8例,P式吻合3例,O式吻合33例。随访结果显示,全组并发症发生率为13.6%,手术死亡率6.8%,5年生存率13.6%,中位生存时间超过12个月。建议:对于Ⅳ期胃癌病例,不应轻易放弃手术治疗,食道空肠吻合方式应以O式吻合术作为首选。
This article reports from 1982 to 1992 in our hospital admitted to Ⅳ gastric total gastrectomy in 44 cases. Roux-en-Y operation was used to reconstruct the digestive tract, including 8 cases of end-to-end esophageal jejunum, 3 cases of P-type anastomosis and 33 cases of O-type anastomosis. The follow-up results showed that the overall incidence of complications was 13.6%, surgical mortality was 6.8%, 5-year survival rate was 13.6%, the median survival time of more than 12 months. Recommendation: For Ⅳ cases of gastric cancer, surgical treatment should not be easily given up, O-type jejunostomy should be O-type anastomosis as the first choice.
