临床观察到听力随耳鸣强度的变化而波动,因此提出是否耳鸣起着类似背景噪声的作用而会影响听力的问題,本文对19例作了全面听力学检查的有耳鸣和听力减退的病人进行主观听阈和ABR阈的比较,另设两组对照,做同样的检查。一组为19例无耳呜、听力正常者,另一组为8例无耳鸣的感音神经聋者。结果发现,耳鸣组的主观听阈值大于客观测听(ABR)阈值,反之,无耳鸣者(不论有无感音神经性听力损失)的主观听阈通常都比ABR阈值为低。耳鸣组ABR阈值比4KHz的主观听力阈值平均低15.0dB,比1、2和4KHz的主观听力阈值平均低10.53dB。其中有4例病人患耳和健耳的ABR阈值差为0~20dB,但其主观听力测试的阈值差则为25~65dB。在本文研究中,耳鸣似乎“影响”主观听阈阈值而不影响ABR的阈
It is clinically observed that the hearing ability fluctuates with the change of tinnitus strength. Therefore, it is proposed whether tinnitus plays a similar role as background noise and will affect the hearing. In this paper, 19 patients with tinnitus and hearing loss who had a comprehensive audiological examination were subjective Hearing threshold and ABR threshold comparison, the other two sets of control, do the same examination. A group of 19 cases without ear sigh, normal hearing, the other group of 8 cases of non-tinnitus sensorineural deafness. The results showed that the subjective listening threshold of the tinnitus group was greater than the objective audiometry threshold (ABR), whereas the subjective hearing threshold of the non-tinnitus (with or without sensorineural hearing loss) was generally lower than the ABR threshold. The tinnitus group had an ABR threshold of 15.0 dB below the subjective hearing threshold of 4K Hz and an average of 10.53 dB below the subjective hearing threshold of 1, 2 and 4 kHz. The ABR threshold difference between the ear and the healthy ear in 4 patients was 0 ~ 20 dB, but the threshold difference of the subjective hearing test was 25 ~ 65 dB.
