目的:初步研究清开灵注射液雾化吸入对食管/贲门癌术后肺部炎症及呼吸功能的影响,为中医药在胸外科围手术期的应用开辟新的思路。方法:选择句容市人民医院胸外科食管/贲门癌手术患者28例,区组随机分为试验组和对照组两组,试验组术后予清开灵注射液雾化吸入,对照组术后予常规雾化液吸入,观察两组用药后体温、血象、胸片改变;术后动脉血气(PaO2、PaCO2、SaO2);咯痰量、痰液性状。并对数据进行统计分析,探讨清开灵注射液雾化吸入对食管/贲门癌术后呼吸功能的影响。结果:试验组术后2~5天咯痰量较对照组比较差异有显著统计学意义(P<0.01),术后第4、5天痰液性状改善较对照组比较差异有统计学意义(P<0.01,P<0.05);两组间术后发热天数比较(P>0.05),第1、6天白细胞计数比较(P>0.05),第1、6天中性粒细胞百分比比较(P>0.05),第1、6天胸片评分比较(P>0.05),第1、3、6天动脉血气比较(PSaO2>0.05、PPaO2>0.05、PPaCO2>0.05),差异无统计学意义。结论:清开灵注射液雾化吸入,能早期稀释食管/贲门癌术后患者痰液,促进患者排痰,但对术后发热情况、肺部炎症状态的改善以及动脉血气影响未优于对照组。
Objective: To study the effect of Qingkailing injection aerosol inhalation on postoperative lung inflammation and respiratory function after esophageal / cardiac cancer operation, and to open up new ideas for the application of traditional Chinese medicine in the perioperative period of thoracic surgery. Methods: Twenty-eight patients underwent thoracic esophageal / cardiac surgery in Jurong City People’s Hospital were randomly divided into experimental group and control group. The experimental group was treated with Qingkailing injection after operation and the control group after operation Inhalation of conventional aerosol fluid, body temperature, hemogram and chest radiograph were observed after treatment. Arterial blood gas (PaO2, PaCO2, SaO2); The data were analyzed statistically to explore the influence of Qingkailing injection aerosol inhalation on postoperative respiratory function of esophagus / cardiac cancer. Results: There were significant differences in the amount of sputum aspiration between 2 and 5 days after operation in the experimental group (P <0.01), and the improvement of sputum traits on the 4th and 5th day after operation compared with the control group (P < P <0.01, P <0.05). The days of postoperative fever in the two groups were compared (P> 0.05), the counts of leukocytes on the first and sixth day were compared (P> 0.05), the percentage of neutrophils on the first and sixth day (P> 0.05). There was no significant difference between the 1st and 6th day of chest radiograph score (P> 0.05) and the arterial blood gas of the 1st, 3rd and 6th day (PSaO2> 0.05, PPaO2> 0.05, PPaCO2> Conclusion: Qingkailing injection atomized inhalation can early dilute esophageal / cardiac cancer patients sputum, and promote sputum in patients with sputum, but postoperative fever, pulmonary inflammatory status improvement and arterial blood gas is not better than the control group.
