目的 探讨2型糖尿病合并非酒精性脂肪肝的患病情况,分析影响2型糖尿病合并非酒精性脂肪肝的相关危险因素.方法 收集2014年1月~2016年7月2型糖尿病患者356例,分为合并脂肪肝组192例与未合并脂肪肝组164例,分析比较两组患者在年龄、病程、体重指数(body mass index,BMI)、是否合并糖尿病视网膜病变、糖化血红蛋白(hemoglobin A1C,HbA1C)、外周血细胞、肝肾功能、空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)、稳态模型胰岛素抵抗指数(homeostasis model assessment insulin resistance,HOMA-IR)、血脂、炎性因子等发面的差异,探讨2型糖尿病合并非酒精性脂肪肝的相关危险因素.结果 在356份资料里,2型糖尿病合并脂肪肝患者占资料总数的53.93%,合并脂肪肝组在BMI、谷丙转氨酶(alanine aminotransferase,ALT)、转肽酶(gamma-glutamyltransferase,GGT)、前白蛋白(Prealbumin,PA)、白蛋白(albumin,ALB)、甘油三酯(triglyceride,TG)、尿酸(uric acid,UA)、HbA1C、FPG、FINS、HOMA-IR、白介素-8(interleukin-8,IL-8)等水平均高于未合并脂肪肝组,差异有统计学意义(P<0.05),在高密度脂蛋白(high density lipoprotein,HDL)水平上低于未合并脂肪肝组,差异有统计学意义(P<0.05).结论 本研究发现2型糖尿病合并脂肪肝患者发病率与BMI、肝肾功能、血脂、HbA1C、FPG、FINS、HOMA-IR、IL-8等因素具有一定的相关性,应采取相关措施预防该疾病的发生和发展.“,”Objective To investigate the prevalence of type 2 diabetes with non-alcoholic fatty liver disease, and to analyze the related factors in type 2 diabetes with non-alcoholic fatty liver disease. Methods 170 patients with type 2 diabetes from January 2015 to July 2016 were recruited and divided into 99 cases with fatty liver group, 71 cases with non-fatty liver group, compared two groups of patients in age, course of disease, body mass index(BMI), diabetic retinopathy, hemoglobin A1C(HbA1C), the peripheral blood cells, liver and kidney function, fasting plasma glucose (FPG),fasting insulin(FINS),homeostasis model assessment of insulin resistance (HOMA-IR),blood lipids and inflammatory factors etc, to analyze the related factors of type 2 diabetes with fatty liver. Results In 170 copies of data , the type 2 diabetes with fatty liver accounted for 58.23%of the total data. The BMI in fatty liver group were higher than those without fatty liver group .The level of Alanine aminotransferase (ALT),Gamma-Glutamyltransferase (GGT), Prealbumin(PA),Albumin(ALB),Triglyceride(TG),Uricacid(UA),HbA1C,FPG,FINS,HOMA-IR,Interleukin-8(IL-8) were significantly higher in fatty group (P< 0.05), While high density lipoprotein(HDL) level was lower than that of non-fatty liver group , the difference was statistically significant (P<0.05). Conclusion:There may be a relationship between BMI, liver and kidney function, blood lipid, HbA1C, FPG,,FINS,HOMA-IR,IL-8 and type 2 diabetes patients with fatty liver, which helps to prevent the occurrence and progress of the disease.
