MRI体素内不相干运动在小型猪颌下腺放疗损伤中的应用价值初步研究

目的探讨体素内不相干运动(IVIM)在小型猪颌下腺放疗损伤中的应用价值。方法将6只雌性广西巴马小型猪随机分成实验组和对照组,将小型猪双侧颌下腺放入照射野内,实验组小型猪照射剂量为20 Gy/每侧颌下腺,对照组小型猪照射剂量为0 Gy/每侧颌下腺。照射前和照射后4周分别对两组小型猪颌下腺行常规MR及双指数模型IVIM扫描,分别观察两组小型猪照射前后以及实验组和对照组之间颌下腺IVIM参数(D值、D~*值和f值)的差异。待MRI检查结束后,处死两组小型猪,取双侧颌下腺标本行HE染色,观察两组小型猪颌下腺组织结构的不同。结果照射前实验组与对照组颌下腺IVIM各参数均无统计学差异,D值分别为(0.855±0.050)×10~(-3)mm~2/s、(0.829±0.041)×10~(-3)mm~2/s;D~*值(18.966±1.323)×10~(-3)mm~2/s、(19.416±1.705)×10~(-3)mm~2/s;f值0.189±0.009、0.194±0.023(P>0.05)。照射后实验组颌下腺IVIM各参数分别为D值(1.007±0.059)×10~(-3)mm~2/s、D~*值(4.788±0.835)×10~(-3)mm~2/s、f值0.394±0.031,与照射前实验组和照射后对照组颌下腺IVIM各参数比较,均存在统计学差异(P<0.05);照射后对照组颌下腺IVIM各参数分别为D值(0.846±0.130)×10~(-3)mm~2/s、D~*值(19.733±3.286)×10~(-3)mm~2/s、f值0.193±0.026,与照射前比较,两者无统计学差异(P>0.05)。实验组与对照组颌下腺病理HE染色结果显示二者明显不同。结论 IVIM可对小型猪颌下腺放疗损伤模型进行无创、量化评估,以期为头颈部放射治疗患者的颌下腺功能损伤评估奠定基础。 Objective To explore the value of non-coherent motion of voxels (IVIM) in the radiotherapy of submaxillary gland in mini-pigs. Methods Six female Bama miniature pigs in Guangxi were randomly divided into experimental group and control group. The bilateral submandibular glands of miniature pigs were placed in the irradiation field. HE staining results of pathological submandibular gland in the experimental group and the control group showed that the two were significantly different. Conclusion IVIM can be used as a noninvasive and quantitative evaluation of miniature pig submandibular gland radiotherapy injury model in order to lay the foundation for the evaluation of submandibular gland function damage in patients with head and neck radiotherapy.
