目的探讨术前区域性动脉灌注化疗对晚期乳腺癌微血管密度(MVD)、微淋巴管密度(MLVD)的影响。方法将76例晚期乳腺癌随机分为两组,术前局部动脉灌注化疗(A组)35例和术前全身静脉化疗(B组)41例,另选25例未行术前化疗的Ⅲ期乳腺癌作对照组。采用免疫组化SP法,用CD34、血管内皮生长因子受体3(VEGFR3)抗体对3组组织标本进行MVD、MLVD检测。结果A组MVD为36.05±13.64,B组为49.92±12.90,对照组为60.38±13.54,各组间比较差异均有高度显著性(P<0.01);A组MLVD为6.62±3.70,B组为9.96±4.57,对照组为11.30±5.32,A组与对照组、B组比较差异有高度显著性(P<0.01),B组与对照组比较差异无显著性(P>0.05)。结论术前区域性动脉灌注化疗能有效地降低肿瘤组织及其周边组织内MVD和MLVD,从而抑制肿瘤细胞生长,减少肿瘤转移的机会。
Objective To investigate the effect of preoperative regional arterial infusion chemotherapy on the microvessel density (MVD) and lymphatic vessel density (MLVD) in patients with advanced breast cancer. Methods Totally 76 patients with advanced breast cancer were randomly divided into two groups. 35 patients underwent local arterial infusion chemotherapy (A group) and 41 patients underwent preoperative systemic intravenous chemotherapy (B group), while another 25 patients undergoing preoperative chemotherapy Breast cancer as a control group. Immunohistochemical SP method was used to detect MVD and MLVD in three groups of tissue samples using CD34 and VEGFR3 antibody. Results The MVD in group A was 36.05 ± 13.64, in group B was 49.92 ± 12.90, in control group was 60.38 ± 13.54, the difference between groups was highly significant (P <0.01); the MLVD in group A was 6.62 ± 3.70, 9.96 ± 4.57 in the control group and 11.30 ± 5.32 in the control group. The difference between the A group and the control group and the B group was highly significant (P <0.01). There was no significant difference between the B group and the control group (P> 0.05). Conclusion Preoperative regional arterial infusion chemotherapy can effectively reduce MVD and MLVD in tumor tissue and its surrounding tissues, thereby inhibiting tumor cell growth and reducing tumor metastasis.
