目的探讨多层螺旋CT(multi-slicespiral CT,MSCT)对胰腺癌血管侵犯的定量评估及可切除性的应用价值。方法对2009年8月至2010年6月吉林大学中日联谊医院行320排CT检查的22例胰腺癌病人进行前瞻性定量评估,以手术结果为“金标准”进行对照分析。结果 MSCT评估血管侵犯的敏感性为91.2%,特异性为98.7%,准确性为96.4%;在测量血管侵犯长度和受侵段血管与血管分叉距离方面仅2支血管测量不准确。结论 MSCT判断胰腺癌血管侵犯的准确性很高,可准确量化血管侵犯的程度及范围,对胰腺癌的诊治有重要的参考价值。
Objective To investigate the quantitative evaluation of resectability of multi-slice spiral CT (MSCT) in the diagnosis of pancreatic cancer. Methods Twenty-two patients with pancreatic cancer who underwent 320-slice CT scan at China-Japan Friendship Hospital of Jilin University from August 2009 to June 2010 were prospectively evaluated quantitatively, and the results of the surgery were analyzed as “gold standard ”. Results The sensitivity, specificity and accuracy of MSCT in assessing vascular invasion were 91.2%, 98.7% and 96.4%, respectively. Only 2 vessels were inaccurate in measuring vessel invasion length and vessel-to-vessel bifurcation distance. Conclusion The accuracy of MSCT in detecting vascular invasion of pancreatic cancer is very high, which can accurately quantify the extent and extent of vascular invasion and provide important reference value for the diagnosis and treatment of pancreatic cancer.
