目的:对彩色B超在胎儿先天性心脏病筛查中应用的临床效果进行分析。方法:选取产检胎心异常的产妇50例,对其临床资料进行分析,筛查时采用的彩超是GE公司生产的VOLUSON730彩超仪。结果:50例胎心异常中单心室13例,占26%;三尖瓣闭锁合并大动脉转位10例,占20%;心内膜垫缺损9例,占18%;心室发育不良6例,占12%;室缺5例,占10%;单心房4例,占8%;法洛四联症3例,占6%。以上诊断经过对照后,诊断错误2例,失误率为4%,原因是由于产妇孕周过大所致。结论:彩色B超对胎儿先天性心脏病筛查诊断优势是明显的,以其可以显示心脏精细结构情况、帮助医师了解胎儿心内血流动力学情况胜于其它筛查手段,包括普通B超。但其价格高昂,技术尚未成熟导致其未能广泛应用,只能用于一些危急或科研情况,有必要培养更多专业人才以推广该项技术。
Objective: To analyze the clinical effect of color B ultrasound in fetal congenital heart disease screening. Methods: Fifty cases of abnormal fetal heart rate were selected and their clinical data were analyzed. The color Doppler ultrasound used by GE was VOLUSON730 Color Doppler. Results: Thirteen cases (26%) had single ventricle in 50 cases of fetal heart failure, 10 cases (20%) had tricuspid valve closure and aortic transposition, 9 cases of endocardial cushion defect (18%), 6 cases of ventricular dysplasia Accounting for 12%; ventricular septum in 5 cases, accounting for 10%; single atrial in 4 cases, accounting for 8%; tetralogy of Fallot in 3 cases, accounting for 6%. After the diagnosis of the above control, the diagnosis of errors in 2 cases, the error rate of 4%, due to maternal gestational age is too large. CONCLUSION: Color B-ultrasound has obvious advantages in the diagnosis of fetuses with congenital heart disease, which can show the fine structure of heart and help doctors to understand the intracardiac hemodynamics over other screening methods, including general B- . However, its high price and immature technology have led to its failure to be widely used. It can only be used in some critical or scientific research situations and it is necessary to train more professionals to promote the technology.
