本文介绍四象限技术——羊水指数(AFI)评价羊水量,以AFI≤5cm为羊水过少。 自1989月8月至1990年4月,本院采用AFI方法诊断羊水过少发生率2.08%(77/3692),其实际发生率为2.05%(76/3692)。由于AFI可以全面评价整个宫腔羊水量,从而提高产前B超诊断羊水过少的准确性和敏感性。 羊水生成受多种因素影响。本组引起羊水过少的各种高危因素26例中宫内发育迟缓占11例,支持宫内慢性缺氧可导致胎儿排尿减少致羊水过少。尚有50例并无其他产科高危因素。为降低围生儿发病率和死亡率,产前全面检测可能对胎儿、新生儿有潜在危险的任何因素,常规B超四象限技术测定羊水量已属必要。本组中AFI有2个或2个以上象限羊水暗区为零者,其Apgar评分均低于7分,提示AFI还可预示胎儿和新生儿的预后,对指导临床处理极有帮助。
This article describes the four quadrant technology - amniotic fluid index (AFI) evaluation of amniotic fluid, AFI ≤ 5cm is too little amniotic fluid. From August 1989 to April 1990, our hospital used AFI method to diagnose the incidence of oligohydramnios 2.08% (77/3692). The actual incidence was 2.05% (76/3692). As AFI can evaluate the entire uterine amniotic fluid volume, so as to improve the accuracy and sensitivity of prenatal B-ultrasound diagnosis of oligohydramnios. Amniotic fluid production is affected by many factors. The group caused by a variety of high risk factors for oligohydramnios 26 cases of intrauterine growth retardation in 11 cases, support for intrauterine chronic hypoxia can lead to decreased fetal urination oligohydramnios. There are still 50 cases of other obstetric risk factors. To reduce the incidence of perinatal morbidity and mortality, prenatal comprehensive detection of possible risk to the fetus, newborn any factor, the conventional B-quadruple-technology to determine the amount of amniotic fluid has been necessary. In this group, AFI has 2 or more than two quadrants amniotic fluid dark area is zero, the Apgar scores were less than 7 points, suggesting that AFI can predict the prognosis of fetus and newborn, to guide the clinical treatment of very helpful.
