目的研究以宁夏两个“人人享有基本医疗卫生服务”政策试点市为样本,比较政策下卫生服务需方和供方,分析运行过程中存在的问题,为基本医疗服务的进一步实施和推广提供政策建议。方法对样本地区进行现场调查,采用一般描述性分析,t检验、χ2检验、TOPSIS法分析比较就诊人次、药事费收入与月份关系,项目运行前后满意度。结果项目实施后村民首选就医地点未发生变化,存在对药品种类及效果不满、不缴纳药事费现象,村卫生室均有药品未到位、药品代领、收入下降等现象。结论通过完善村卫生室配套制度,加强药品配送单位的督管,转变贵药,输液才能治病的观念,提高收入及简化工作量,加强基层医疗机构领导能力及村医医德建设等措施提高了项目满意度。
Objective To study two pilot cities of Ningxia on “basic medical and health services for all” as a sample, to compare the demand side and supply side of health service under the policy, to analyze the existing problems in the operation process, to further implement the basic medical services and Promote the provision of policy advice. Methods Field survey was conducted in the sample area. Results After the implementation of the project, the villagers preferred the place of medical treatment did not change, there is dissatisfaction with the type of drug and effect, do not pay the cost of drug fees, village clinics are not in place of medicines, drugs on behalf of the collar, the decline in income and so on. Conclusion By improving the supporting system of village clinics, strengthening the supervision of drug delivery units, changing the concept of expensive medicine and transfusion to cure diseases, increasing income and simplifying workload, measures such as strengthening the leadership ability of village-level medical institutions and building the village medical ethics are improved Project satisfaction.
