目的探究颌间牵引钉联合微型钛板内固定修复颌骨骨折患者疗效。方法选取石河子大学医学院第一附院2013年6月~2015年5月收治的82例颌骨骨折患者,依据治疗方法不同分为对照组与研究组,各41例。对照组实施微型钛板内固定治疗,研究组实施颌间牵引钉与微型钛板内固定联合治疗。术后随访半年,统计两组手术用时、临床疗效、骨折愈合时间、不良反应情况,并对比两组治疗前后牙周情况相关指标[菌斑指数(PI)、软垢指数(DI)、牙龈指数(GI)]评分情况。结果两组手术用时比较,差异无统计学意义(P>0.05);与对照组相比,研究组骨折愈合时间较短,差异有统计学意义(P<0.05);两组治疗优良率相比,研究组97.56%(40/41)远高于对照组80.49%(33/41),差异有统计学意义(P<0.05);治疗前,两组PI、DI、GI差异无统计学意义(P>0.05);治疗后,与对照组相比,研究组PI、DI、GI均较低,差异有统计学意义(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论颌间牵引钉与微型钛板内固定联合应用于颌骨骨折治疗中,可有效提高治疗性优良率,显著缩短骨折愈合时间,且对牙周组织影响较小,安全性较高。
Objective To investigate the effect of intermaxillary traction nail and mini titanium plate fixation for the treatment of patients with jaw fracture. Methods 82 cases of maxillofacial fractures admitted from June 2013 to May 2015 in First Affiliated Hospital of Shihezi University were divided into control group and study group, 41 cases in each group. The control group was treated with mini-titanium plate internal fixation, and the research group performed intermaxillary traction nail and micro-titanium plate fixation. The patients were followed up for six months. The operative time, clinical curative effect, fracture healing time and adverse reactions were compared between the two groups before and after treatment. The related indexes of periodontal status were compared before and after treatment [plaque index (PI), soft dirt index (DI) (GI)] rating. Results There was no significant difference in operative time between two groups (P> 0.05). Compared with the control group, the fracture healing time of the study group was shorter and the difference was statistically significant (P <0.05) (97.56%, 40/41) in the study group were significantly higher than that in the control group (80.49%, 33/41) (P <0.05). Before treatment, there was no significant difference in the PI, DI, P> 0.05). After treatment, compared with the control group, the PI, DI, GI in the study group were lower, the difference was statistically significant (P <0.05); There was no significant difference in the incidence of adverse reactions between the two groups (P> 0.05). Conclusion Intermaxillary traction nail combined with mini-titanium plate fixation in the treatment of jaw fracture can effectively improve the therapeutic rate, shorten the fracture healing time significantly, and have little effect on periodontal tissue and have higher safety.
