目的评估搏动性导管(pulsatilecatheter,PUCA)泵对急性心力衰竭绵羊辅助的有效性和安全性。方法对10只绵羊诱导左心衰竭模型建立成功后行PUCA泵辅助3h,监测并记录血流动力学参数,采动脉血人工计数血小板并测定血浆游离血红蛋白;经辅助3h后检查肾、隔膜泵及导管血栓形成的情况。结果10只绵羊中7只成功支持血循环达到规定的时间,另3只绵羊因技术原因未能完成实验。成功的7只绵羊在支持期间血流动力学逐步恢复并趋向稳定,血压接近基线值;血小板和血浆游离血红蛋白在辅助期间无明显变化;肾、隔膜泵及导管均未见明显的血栓形成。结论PUCA泵的短期辅助能稳定急性心力衰竭动物的血流动力学,并且无明显的血小板和红细胞破坏及血栓形成等并发症。因此,其应用安全有效,可用于临床心力衰竭患者的短期支持。
Objective To evaluate the effectiveness and safety of pulsatilecatheter (PUCA) pumps in assisting sheep with acute heart failure. Methods The model of left heart failure induced by 10 sheep was established successfully, then the PUCA pump was used to assist for 3 hours. The hemodynamic parameters were monitored and recorded. Arterial blood was collected to measure platelets and plasma free hemoglobin. After 3 hours, Catheter thrombosis. Results 7 out of 10 sheep successfully supported blood circulation for the prescribed time, and the other 3 sheep failed to complete the experiment due to technical reasons. The success of 7 sheep during hemodynamic recovery and gradually stabilized and stabilized, blood pressure close to baseline; platelet and plasma free hemoglobin in the auxiliary period no significant change; kidney, diaphragm pump and catheter showed no significant thrombosis. Conclusion The short-term adjuvant of PUCA pump can stabilize the hemodynamics of acute heart failure animals without any obvious complications such as platelet and erythrocyte destruction and thrombosis. Therefore, its application is safe and effective and can be used for the short-term support of patients with clinical heart failure.
