Objective To explore the importance of using trigeminal nerve microvascular decompression in the treatment of primary trigeminal neuralgia to avoid and retain petrous vein. Methods Twenty-four patients with primary trigeminal neuralgia were treated with microvascular decompression to preserve the petrosal vein and the clinical effect was observed. Results Twenty-one patients (87.5%) had early pain relief after operation and 3 patients (12.5%) had delayed discharge at discharge. One patient recurred (4.2%) after 4 months to 1 year follow-up. None of the 24 patients Complications occur. Conclusions The use of microvascular decompression to preserve the petrosal vein in the treatment of trigeminal neuralgia can significantly improve the curative effect, avoid the intraoperative injury, and can effectively reduce the occurrence of complications.
