为研究经脐动脉和脐静脉建立体外循环的可行性 ,解剖 90个新鲜足月新生儿脐带 ,发现其中 86个脐带由 2根脐动脉和 1根脐静脉组成 ,4个脐带由 1根脐动脉和 1根脐静脉组成 ;测量单脐动脉、双脐动脉和脐静脉的压扁内壁周径与外壁周径分别是 3 8±0 3mm与 6 2± 0 3mm、2 4± 0 2mm与 4 6± 0 2mm和 10 0± 0 4mm与 10 8± 0 4mm ;单脐动脉和双脐动脉在 80mmHg充盈压下的直径分别是 4 8± 0 1mm和 3 9± 0 8mm ,在 2 0mmHg压力下脐静脉的直径是 7 2± 0 3mm。据此测量结果设计、制作内径 4 0~ 8 0mm的金属插管并应用于 1例出生时心跳骤停的患儿 ,成功地经脐动静脉建立了体外循环。结果提示 ,经脐带建立体外循环简单易行、创伤小、有实用价值
In order to study the feasibility of establishing cardiopulmonary bypass through umbilical artery and umbilical vein, 90 fresh full-term newborn umbilical cord were dissected and found that 86 umbilical cord consisted of two umbilical arteries and one umbilical vein, and four umbilical cord belonged to one umbilical artery And 1 umbilical vein. The measurements of the crushed wall diameter and the peripheral wall diameter of single umbilical artery, double umbilical artery and umbilical vein were 38 ± 0 3mm and 6 2 ± 0 3mm, 2 4 ± 0 2mm and 4 6 respectively ± 0 2mm and 10 0 ± 0 4mm and 10 8 ± 0 4mm respectively. The diameters of single umbilical artery and double umbilical artery under pressure of 80mmHg filling were 48 ± 0 1mm and 39 ± 0 8mm, respectively. Under the pressure of 20mmHg, The diameter of the vein is 7 2 ± 0 3mm. According to the results of the design, a metal cannula with a diameter of 40 ~ 80 mm was made and applied to 1 patient with cardiac arrest at birth. Cardiopulmonary bypass was successfully established through the umbilical artery and vein. The results suggest that the establishment of extracorporeal circulation through the umbilical cord is simple, less trauma, and has practical value
