目的探讨超声双重造影检查术前诊断胃癌分期判定中的临床价值。方法选取在本院进行手术治疗的114例胃癌患者进行回顾性分析,所有患者术前接受胃窗超声造影、双重超声造影检查,以术后病理学结果作为TNM分期金标准,比较两种超声造影检查术前诊断胃癌分期的价值。结果胃窗造影检查诊断T_1期14例、T_2期26例、T_3期40例、T_4期34例,其中诊断级别升高12例、诊断级别降低12例,正确诊断分期90例,诊断准确率78.95%,与病理结果的一致性kappa值为0.708(P<0.001);超声双重造影检查诊断T_1期14例、T_2期25例、T_3期41例、T_4期34例,其中诊断级别升高6例、诊断级别降低6例,正确诊断分期102例,诊断准确率89.47%,与病理结果的一致性kappa值为0.844(P<0.001);胃窗造影检查诊断术前诊断胃癌分期的准确率78.94%显著的低于超声双重造影检查的89.47%(P<0.05)。结论超声双重造影检查术前诊断胃癌分期与术后病理学的一致性较高,具有一定的临床价值。
Objective To investigate the clinical value of double-contrast ultrasound in the diagnosis of gastric cancer staging. Methods A retrospective analysis was performed on 114 gastric cancer patients undergoing surgical treatment in our hospital. All patients underwent preoperative gastric window ultrasonography and double contrast echocardiography. Postoperative pathological results were used as the gold standard for TNM staging. Results Gastroesophageal angiography was diagnosed in 14 cases of T_1, 26 cases of T_2, 40 cases of T_3 and 34 cases of T_4, including 12 cases with elevated diagnostic level, 12 cases with lower diagnostic level, 90 cases with correctly diagnosed stage, and the diagnostic accuracy was 78.95 %, Consistent with the pathological results kappa value of 0.708 (P <0.001); double contrast-enhanced ultrasound diagnosis of T_1 in 14 cases, T_2 in 25 cases, T_3 in 41 cases, T_4 in 34 cases, of which the diagnostic level increased in 6 cases , The diagnostic level was reduced in 6 cases, the correct diagnosis of staging in 102 cases, the diagnostic accuracy rate of 89.47%, and the consistency of the pathological results kappa value of 0.844 (P <0.001); gastric window imaging diagnosis of gastric cancer staging accuracy of 78.94% Significantly lower than 89.47% (P <0.05) of ultrasound double contrast examination. Conclusion The double-contrast ultrasonography preoperative diagnosis of gastric cancer staging and postoperative pathology consistency is high, with a certain clinical value.
