目的:分析孕产妇死亡的原因及其主要影响因素,为降低孕产妇死亡率而提供科学依据。方法:采用回顾性分析的方法,对重庆市万州区妇幼保健院2006年至2015年孕产妇死亡病例资料及评审结果进行分析。结果:2006年至2010年以产科出血、妊娠期高血压引起的直接产科原因为主,2011年至2015年以妊娠合并心脏疾病、肝脏疾病等内科合并症引起的间接产科原因为主。小学以及小学以下学历者占总孕产妇死亡人数的53.85%,初中学历占总人数的26.92%以上,高中及其以上占19.23%左右;产检次数<5次占57.69%,产检0次占15.35%,产检次数≥5次占26.92%。孕产妇死亡主要与医疗保健机构知识技能(57.69%)、态度(15.38%)、管理(15.38%)有关,与个人知识技能(46.15%)、资源(34.62%)态度(11.54%)有关。结论:孕产妇死亡跟社会、医学,家庭以及个人文化素养有着密切的关系,降低孕产妇死亡率,需要社会多方面和多学科的合作,方能取得显著成效。加强医疗监管、落实贫困孕产妇危重症大额救助和提高医务人员的知识与技能水平是降低孕产妇死亡率的关键。
Objective: To analyze the causes of maternal death and its main influencing factors, and provide scientific evidence for reducing maternal mortality. Methods: A retrospective analysis of maternal and child health centers in Wanzhou District of Chongqing from 2006 to 2015 cases of maternal deaths and assessment results were analyzed. Results: The main obstetric causes were obstetric bleeding and gestational hypertension from 2006 to 2010. From 2011 to 2015, indirect obstetric causes were the cause of medical complications such as heart disease and liver disease during pregnancy. Primary education and primary education accounted for 53.85% of the total number of maternal deaths, junior high school education accounted for more than 26.92% of the total population, high school and above accounted for about 19.23%; births less than 5 times accounted for 57.69%, 0 times accounted for 15.35% , The number of births ≥ 5 times accounted for 26.92%. Maternal death was mainly related to knowledge and skills (57.69%), attitude (15.38%) and management (15.38%) in health care institutions, and personal knowledge and skills (46.15%) and resources (34.62%) (11.54%). Conclusion: Maternal mortality is closely related to social, medical, family and individual cultural attainment. To reduce maternal mortality rate, multi-faceted and multidisciplinary cooperation in the community is needed to achieve remarkable results. Strengthening Medical Supervision and Implementing Maternity Males Critical assistance and raising the level of knowledge and skills of medical staff are the keys to reducing maternal mortality.
