为探讨围体外循环期(PCPB)血浆内皮素(PET)与肾素(PRA)、血管紧张素Ⅱ(ANGⅡ)、醛固酮(ALD)的变化及关系,研究了三组肺循环压力不同(以右室流出道压代表循环压力,分为轻、中度肺循环压力升高组,压力分别为4~8kPa、8~12kPa,记为mPP及MPP组及肺循环压力降低组,记为DPP组)。结果表明:PET在CPB期间均降低,以mPP组最著;术后均升高,以MPP为著,组间差异均有显著性(P<0.05)。PRA在CPB期升高明显,以mPP组最著;术后降低,但仍高于术前,以MPP组为著,组间差异均有显著性(P<0.01)。ANGⅡ、ALD的变化趋势同PRA。相关分析表明:DPP组在CPB30min、术后12h、24h,PET与PRA间均呈显著正相关(r及P依次为0.77,<0.05;0.76,<0.05;0.82,<0.01),PET与ANGⅡ在术后12h、24h亦呈显著正相关(r及P依次为0.78,<0.05;0.89,<0.01),在MPP组PET与尿量在CPB期间呈显著负相关(r=-0.80,P<0.01)。PET与术后引流量之间三组病人均无明确相关关系。推测先天性心脏病人PCPB期PET与R?
To investigate the changes of plasma endothelin (PET) and renin (PRA), angiotensin Ⅱ (ANGⅡ) and aldosterone (ALD) during the cardiopulmonary bypass (PCPB) Outflow channel pressure on behalf of the circulatory pressure, divided into light and moderate pulmonary pressure group, the pressure was 4 ~ 8kPa, 8 ~ 12kPa, recorded as mPP and MPP group and pulmonary pressure reduction group, recorded as DPP group). The results showed that: PET decreased during CPB, with mPP group the most; postoperative were elevated, with MPP as the significant difference between the two groups (P <0.05). PRA increased significantly in CPB stage, the most in mPP group; decreased after operation, but still higher than preoperative, as in MPP group, there was significant difference between groups (P <0.01). ANG Ⅱ, ALD trend with PRA. Correlation analysis showed that there was a significant positive correlation between PET and PRA at DPP30min, 12h, 24h after PCI (r = 0.77, <0.05; 0.76, <0.05; 82, <0.01). There was also a significant positive correlation between PET and ANGⅡ at 12h and 24h after operation (r and P were 0.78, <0.05; 0.89, <0.01) There was a significant negative correlation between PET and urine volume during CPB (r = -0.80, P <0.01).
