目的:探讨人巨细胞病毒(Human cytomegalovirus,HCMV)宫内感染对新生儿行为神经发育的影响。方法:选择胎龄37-42周、脐血或外周血检查HCMV IgM阳性、母亲产前筛查或产时监测HCMV IgM阳性的24例新生儿作为感染组,同期检查母亲及新生儿脐血或外周血HCMV感染均为阴性的100例新生儿为对照组,两组新生儿均在出生后48-72 h、12-14天、26-28天测查NBNA评分。结果:新生儿HCMV宫内感染率为1.48%;48-72 h、12-14天、26-28天三个时间段,HCMV感染组患儿的行为能力、被动肌张力及主动肌张力评分与对照组新生儿比较差异均有统计学意义(P<0.05),原始反射评分与对照组比较差异均无统计学意义(P>0.05),一般评估评分在48-72 h差异有统计学意义(P<0.05),而在12-14天、26-28天评分均为满分,与对照组比较无显著差异;三个时间段,HCMV感染组新生儿NBNA评分总分均低于对照组(P<0.05)。结论:HCMV宫内感染可影响新生儿行为神经的发育,主要导致行为能力及主被动肌张力的下降。
Objective: To investigate the effect of human cytomegalovirus (HCMV) intrauterine infection on the behavioral neurogenesis in neonates. Methods: Twenty-four newborns with positive HCMV IgM, or prenatal screening or HCMV-IgM during pregnancy were selected as gestational age 37-42 week. Umbilical cord blood or peripheral blood was used as the infection group. The mothers and newborns’ One hundred newborns with negative HCMV infection were selected as the control group. NBNA score was measured at 48-72 h, 12-14 d and 26-28 d after birth in both groups. Results: The intrauterine infection rate of HCMV in neonates was 1.48%.
