目的研究乳腺癌化疗后患者的人格和生活质量变化以及与癌因性疲乏相关性。方法研究对象选取上海市肿瘤医院和中山医院2013年12月到2014年7月间接受化疗治疗的乳腺癌患者66例,应用横断面调查方法,采用一般资料问卷、癌因性疲乏量表(CFS)、症状自评量表(SCL-90)及艾森克人格问卷简式量表(EPQ-RSC)对患者进行调查,分析化疗对患者的CFS、SCL-90及EPQ-RSC评分影响,并用Spearman相关分析其相关性。结果所有患者的疲乏总分为(35.45±7.21)分,处于中度水平;神经质和精神质评分均名高于国内常模标准得分(P<0.05);化疗后所有患者的SCL-90总分和各因子得分均明显高于化疗前(P<0.01);神经质与疲乏总分、躯体疲乏、心理疲乏、认知疲乏均存在显著正相关(P<0.01),与SCL-90总分、抑郁、焦虑和恐怖因子呈正相关(P<0.01);精神质与情感疲乏存在显著正相关(P<0.01);内-外向性与疲乏总分、心理疲乏均存在显著负相关(P<0.01),与抑郁、焦虑和恐怖因子呈负相关(P<0.01)。结论癌因性疲乏较普遍存在于乳腺癌化疗患者,化疗在一定程度上危害患者心理健康。癌因性疲乏可能受人格因素影响,而心理健康与人格也可能存在一定相关性。
Objective To study the changes of personality and quality of life in patients with breast cancer after chemotherapy and its relationship with cancer-related fatigue. Methods Subjects Sixty-six patients with breast cancer who underwent chemotherapy from December 2013 to July 2014 in Shanghai Cancer Hospital and Zhongshan Hospital were enrolled in this study. The general data questionnaire and cancer-related fatigue scale (CFS (SCL-90) and EPQ-RSC were used to investigate the effect of chemotherapy on CFS, SCL-90 and EPQ-RSC scores of patients Spearman correlation analysis of its relevance. Results The total fatigue score of all patients was (35.45 ± 7.21) at moderate level. The score of neuroticism and psychoticism was higher than that of the national standard (P <0.05). The score of SCL-90 in all patients after chemotherapy (P <0.01). There was a significant positive correlation between neuroticism and total score of fatigue, somatic fatigue, mental fatigue and cognitive fatigue (P <0.01) , Anxiety and horror factors (P <0.01). There was a significant positive correlation between mental quality and emotional exhaustion (P <0.01). There was a significant negative correlation between extroversion and total fatigue score and mental fatigue (P <0.01) Negatively correlated with depression, anxiety and terrorist factors (P <0.01). Conclusion Cancer-related fatigue is more common in patients with breast cancer chemotherapy, chemotherapy to a certain extent, endanger the mental health of patients. Cancer-related fatigue may be affected by personality factors, and mental health and personality may also be some relevance.
